推拿干预对周围神经损伤大鼠脊髓背角中NTFs表达变化的研究

摘要
[目的]
     基于推拿治疗良好的临床疗效,内源性神经营养因子对损伤神经有修复作用,拟从实验角度验证推拿是否可通过促进内源性神经营养因子的表达而促进损伤神经的修复及神经元-神经-肌肉循环通路的确立,为进一步深入探究推拿起效机理及作用途径奠定基础。【方法】
     本课题以清洁级SD大鼠为实验动物,采用坐骨神经夹持损伤模型,随机分为正常组、模型组、模型对照组、推拿治疗组,以按摩推拿手法模拟仪进行干预,斜板试验、耐痛阈、痛敏分数观察行为学指标改变;HE染色光镜观察肌细胞直径、脊髓神经元等形态学改变;采用SABC免疫组织化学方法观察神经营养因子表达的变化。【结果】
     斜板试验、腓肠肌湿重、腓肠肌恢复率,推拿治疗组优于模型对照组(p<0.05);
     耐痛阈,痛敏分数,推拿治疗组均小于模型对照组(p<0.0 5);
     光镜观测肌细胞直径,推拿治疗组大于模型对照组(p<0.05);脊髓神经元存活数量,推拿治疗组多于模型对照组;
     免疫组化染色,与模型组相比较,推拿治疗组可有效促进机体NGF、bFGF分泌(p<0.05),对BDNF有一定促进分泌的作用,但差别不明显;对CNTF表达促进作用不明显,有待进一步研究。【结论】
     推拿对于治疗周围神经损伤有确切的疗效。推拿治疗可有效促进神经损伤后机体NGF、bFGF分泌,影响BDNF、CNTF表达,从而起到保护神经元,促进受损神经修复与再生;刺激神经支配靶器官(肌肉)的结构及功能的恢复;加速了神经元-神经-靶器官循环通路的恢复,促进了受损神经再生的协调性、全面性。
【Objective】
     Based on the good curative effect of traditional chinese massage therapy, the repairing function of endogenous neurotrophic factors to the damage nerves, take experiments to verified massage could shorted the repairing period of the damaged nerve by promoting endogenous neurotrophic factors expression and the cycle pathways of neurons nerve-muscle for the further explore established massage effect mechanism and function approach lay the foundation.【Methods】
     The study take SD rats as experimental animals, the model of sciatic nerve injured by clamping, randomly divided into normal group, model group, model control group, massage therapy group, using the massage simulator to intervene, observed behavioral indicators of change: Rivlin' oblique plate test, heat pain threshold, Pain sensitive scores; Morphological changes such as the muscle cell diameter, the shape of spinal cord neurons and so on; the expression of nerve growth factor changes by SABC immunohistochemical【Result】
     Rivlin' oblique plate test, gastrocnemius wet weight, gastrocnemius muscle recovery rate, massage therapy group were better than in model control group (p<0.05);
     The paw withdrawal thermal latency, Pain sensitive scores, massage therapy group were less than model group (p<0.05);
     The diameter of muscle cells observed by light microscopy, massage therapy group were higher than model group (p<0.05); the number of surviving neurons in the spinal cord, massage therapy group were larger than the model control group;
     Immunohistochemical staining, compared with model group,massage therapy group can effectively promote the body NGF, bFGF secretion (p <0.05), has some effect in promoting the secretion of BDNF, but the difference between message group and model group was not significant; role in promoting the expression of CNTF is not obvious, need a further study. 【Conclusions】
     There is a genuine effect of the treatment of peripheral nerve injury by massage, massage therapy can effectively promote the endogenous neurotrophic factors expression such as NGF、bFGF,affect the expression of BDNF, CNTF, the massage thereby protecting neurons in peripheral nerves, and promote repairing and regeneration of the injured nerves; stimulate regeneration and functional recovery of the target organ (Muscle) administrated by nerve; accelerated neuronal nerve-target organ recovery pathway to improve the coordination of damaged nerve regeneration comprehensive.
