皮肤热疗方法治疗皮炎的研究

摘要
研究背景
     近年来,物理疗法治疗皮肤病以其操作简便、价格低廉、副作用小等优势受到患者的欢迎。热疗法用于各类疾病的治疗更是以其安全、便捷的特点得到了患者的肯定。
     皮肤病的致病原多为病毒、细菌、寄生虫等,杀灭皮肤病的致病原是治愈皮肤病的关键。如世界性的流行病血吸虫病初期,血吸虫尾蚴侵入哺乳动物皮肤后会引发尾蚴性皮炎,而血吸虫尾蚴为热敏感生物,因此,设想通过对患处进行皮肤热疗仪热疗来治愈尾蚴性皮炎,其原理是利用热杀死血吸虫尾蚴。
     有报道,48℃热疗联合低温等离子体法治疗痤疮可取得良好的效果。热疗应用于痤疮的治疗已有报道;国内外科学家研究表明低温等离子体可用于各种细菌的杀灭,同时还有促进创伤愈合的作用。由于重度痤疮的产生和发展多与以痤疮丙酸杆菌为首的复合细菌感染有关,因此抑制细菌的生长并杀灭细菌成为解决问题的关键。理论上讲,具有抑制细菌生长,杀灭细菌并促进创伤愈合以及组织再生作用的操作可用于痤疮的治疗。
     本研究基于上述设想,主要研究本实验室自行开发制作的皮肤热疗仪治疗尾蚴性皮炎的可行性,以及48℃热疗联合低温等离子体用于痤疮治疗的可行性。拟解决热疗治疗尾蚴性皮炎是否可行,低温等离子体对活体组织是否安全,热疗仪对皮肤的热辐射范围以及48℃热疗联合低温等离子体对痤疮的治疗是否可行问题。并为皮肤热疗仪用于其他皮肤病的治疗提供依据。
     因此,本研究以上述几个问题为基础,设计模型实验和动物实验,欲得出皮肤热疗仪在哺乳动物体内的升温曲线、观察血吸虫尾蚴的形态和生命活动、评价等离子体作用于健康兔耳的安全性。以及设计6组动物实验,通过比较来探索48℃热疗联合低温等离子体对痤疮的治疗的可行性。
     实验目的
     1、研究皮肤热疗仪在哺乳动物皮下不同深度的升温情况,绘制出皮肤热疗仪不同温度在哺乳动物体内不同深度的升温曲线。
     2、观察血吸虫尾蚴的形态和生命活动。
     3、评价低温等离子体辉光用于皮肤病治疗的安全性。
     4、研究48℃热疗联合等离子体法对兔耳角化模型的治疗效果,为热疗法和等离子体疗法治疗皮肤疾病的临床实验及应用提供科学依据。
     实验方法
     1、皮肤热疗仪对琼脂体模和大耳白兔组织的加温实验:使用皮肤热疗仪分别对琼脂体模和大耳白兔组织表皮进行加热,得到皮肤热疗仪调至不同温度时,皮下不同深度的升温情况,为本研究治疗实验提供数据依据,并作为临床使用热疗法治疗皮肤疾病的参考。
     2、血吸虫尾蚴的形态和生命活动观察实验:使用荧光染料对血吸虫尾蚴进行活体染色,通过低熔点琼脂糖固定,并在激光共聚焦显微镜下观察血吸虫形态;在光学显微镜下和激光共聚焦显微镜下观察血吸虫尾蚴的生命活动。
     3、等离子体辉光作用于健康兔耳的安全性评估实验:使用等离子体发生装置对健康兔耳连续进行辉光处理7天,进行肉眼观察记录,并于第8天取耳片,病理切片并于电镜下观察。根据结果评价等离子体作用于健康兔耳的安全性。
     4、热疗联合等离子体法对兔耳痤疮角化模型的治疗实验:制造大耳白兔的兔耳痤疮角化模型,造模成功后将大耳白兔分为6组:空白对照组、模型对照组、维A酸软膏处理组、热疗法处理、等离子体疗法处理组、热疗联合等离子体疗法处理组进行处理,通过比较实验结果来评价热疗和等离子体法对兔耳痤疮角化模型的治疗效果。
     实验结果
     1、皮肤热疗仪对琼脂体模和大耳白兔组织表皮的加温实验:得到热疗仪调至48℃、50℃、52℃、54℃时,大耳白兔皮下各深度的升温曲线;模拟出了大耳白兔皮下1mm、2mm、3mm、4mm、5mm和6mm处随着表面热疗触头温度升高的升温曲线。
     2、在普通显微镜和激光共聚焦显微镜下均清晰观察到血吸虫尾蚴的形态和生命活动,并得到图像。
     3、等离子体辉光作用于健康兔耳的安全性评估实验:观察记录结果和病理切片结果表明,未观察到兔耳组织发生变化,本实验用强度的等离子体辉光对于兔耳组织安全,没有伤害。
     4、热疗联合等离子体法对兔耳痤疮角化模型的治疗实验:对比6组实验结果表明,热疗法和等离子体疗法均对兔耳痤疮角化模型具有一定的治疗作用。治疗组中,疗效最好的为热疗联合等离子体疗法,其次为热疗组,两组疗效略优于临床治疗痤疮的常用药维A酸软膏。
     结论
     皮肤热疗仪作用于大耳白兔组织表皮后,温度可有效传导至皮下一定深度,并呈现一定规律性,荧光标记法观察血吸虫可行,热疗能否治愈尾蚴性皮炎有待动物实验进一步验证;实验用强度的等离子体辉光对兔耳无伤害;48℃热疗联合等离子体疗法对兔耳痤疮角化模型具有较明显的治疗作用。
Objective
     To research the warming conditions of skin hyperthermia apparatus at different depths in the mammalian skin, to evaluate the safety of the plasma glow treatment for skin,to research the therapeutic effect of hyperthermia combined with plasma treatment for acne keratosis model of rabbit ears.To provide a scientific basis for the using of hyperthermia and plasma for treatment of skin diseases.
     Methods
     1.Heat Agar phantom and skin of rabbits buttock by using skin hyperthermia apparatus:Heat Agar phantom and skin of rabbits buttock respectively by using skin hyperthermia apparatus.To get the warming conditions at different depths in the rabbit skin and provide a reference of skin hyperthermia in clinical trials.
     2.Assess the the safety of Plasma glow effect on the health rabbit ear:Plasma glow the health rabbit ear by using Plasma generator for seven consecutive days,get the rabbit ear tissue On the eighth day, then histopathological analysis was performed(HE staining).It's biological security was assessed according to the results.
     3.Hyperthermia combined with plasma treatment for acne model of rabbit ears:makeacne keratosis model of rabbit ears and be sure the model has been created.then the rabbits were divided into six groups:control group,model group,group of retinoic acid,group of hyperthermia,group of plasma treatment,group of hyperthermia combined with plasma treatment.Evaluated effect of hyperthermia combined with plasma treatment for acne keratosis model of rabbit ears by comparing the results.
     Result
     1.Heat Agar phantom and skin of rabbits buttock by using skin hyperthermia apparatus:got the curves which express warming conditions at different depths in the rabbit skin,when the temperature of skin hyperthermia apparatus is 48℃,50℃,52℃and 54℃. And simulated the curves which express warming conditions when skin heated by hyperthermia apparatus,at different depths in the rabbit skin,for example,1mm,2mm,3mm,4mm,5mm,7mm under the skin.
     2.Assess the the safety of plasma glow effect on the health rabbit ear:observed and recorded the results,combined with histopathological analysis,concluded that plasma glow is safe to the health rabbit ear.
     3.Hyperthermia combined with plasma treatment for acne model of rabbit ears:compare six experimental groups,both hyperthermia and plasma treatment have certain therapeutic effects for acne keratosis model of rabbit ears.In four grounps of treatment,group of hyperthermia combined with plasma treatment reflect the best effect.the second one is group of hyperthermia. The effects of both groups are better than group of retinoic acid which is in common use for acne.
     Conclusion
     1.When skin of rabbits buttock was heated by using skin hyperthermia apparatus,temperture at different depths in the mammalian skin increased simultaneously.What's more,they are regularly.
     2.Plasma glow for experiment is safe to the health rabbit ear.
     3.Hyperthermia combined with plasma treatment has visible therapeutic effect for acne keratosis model of rabbit ears.
