血浆脂联素水平、PWV与冠心病的相关性研究

摘要
目的
     探讨血浆脂联素水平、脉搏波速(PWV)在冠心病患者中的变化,以及二者与冠心病的临床分型、病变程度的关系,并探讨二者之间的关系及临床意义。
     方法
     本研究分冠脉组和对照组,65例冠心病患者均选自2009年6月-2011年6月期间本院心内科住院病人,对照组从2130位健康体检者筛选出且冠脉造影证实冠脉血管正常,无其他原因心血管疾病,无高血压及糖尿病,各项检测指标均在正常范围。采用酶联免疫法检测血浆脂联素的水平,动脉硬化检测仪检测PWV。分别根据冠心病临床分型、严重程度、PWV值进行分组,比较各组间的关系。
     结果
     .165例冠脉组和65例对照组,测得血浆脂联素水平,均数±标准差分别是6.26±2.01,9.39±2.56,两组差异有统计学意义(P<0.05);
     2.65例冠心病组,稳定型心绞痛有31例,占47.69%,血浆脂联素8.98±2.56,不稳定型心绞痛有24例,占36.92%血浆脂联素6.02±2.03,心肌梗死有10例,占15.38%,血浆脂联素4.56±1.99,每两组进行t检验,P均小于0.05,差异有统计学意义;
     3.65例冠心病组,一支病变有21例,占32.30%,血浆脂联素7.98±2.16,二支病变有25例,占36.46%,血浆脂联素5.02±1.98,三支病变有19例,占29.23%,血浆脂联素3.56±2.33,如表3.3所示,每两组进行t检验,P均小于0.05,差异有统计学意义;
     4.65例冠脉组和65例对照组,测得baPWV值分别是15.27±1.70m/s、12.34±2.50m/s,进行两独立样本的均数比较,P值<0.05,差异有统计学意义;
     565例冠心病组,稳定型心绞痛有31例,占47.69%,baPWV值14.01±2.16,不稳定型心绞痛有24例,占36.92%baPWV值15.02±1.03,心肌梗死有10例,占15.38%,baPWV值15.56±1.39如表3.3所示,每两组进行t检验,P均大于0.05,差异无统计学意义。
     6.65例冠心病组,一支病变有21例,占32.30%,baPWV值为13.78±2.26,二支病变有25例,占36.46%,baPWV值15.20±2.58,三支病变有19例,占29.23%,baPWV值17.46±2.43,每两组进行t检验,P均小于0.05,差异有统计学意义;
     7.冠心病患者baPWV平均测定值为15.27±1.70,将测量值在14.00(m/s)以下的作为低baPWV组,14.00(m/s)以上的作为高baPWV组,其中低baPWV组36例,高baPWV组29例,比较低baPWV组和高baPWV血浆脂联素的水平,进行两独立样本的均数比较,P值<0.05,差异有统计学意义;
     结论
     1.血浆脂联素、PWV与冠心病有关,冠心病患者血浆脂联素水平较非冠心病患者低,PWV值较非冠心病患者高;
     2.血浆脂联素水平与冠心病的临床类型以及病变程度有关,随着冠状动脉病变程度和范围的增加,血浆脂联素浓度逐渐下降,PWV与冠心病的病变程度有关,随着病变程度的加重而渐高;
     3.冠心病患者PWV值越高,脂联素水平越低,二者有协调作用。
Objective
     Explore plasma adiponectin, pulse wave velocity (PWV) changes in patients with coronary heart disease, further discussing both with coronary heart disease clinical classification of lesions and the degree of relationship, and to explore the relationship between them and clinical significance.
     Methods
     The study points coronary heart disease group and control group,65cases of coronary heart disease patients were selected from June2009-June2011during our heart medicine hospital patients, from2130healthy check-up in the control group were those with angiographically demonstrated coronary arteries and normal, no other causes of cardiovascular disease, hypertension, and diabetes without, the detection of the indexes in the normal range. The enzyme league immune method to detect the plasma adiponectin levels, arteriosclerosis detector PWV detection. According to the clinical classification of coronary heart disease, serious degree, PWV group value, this paper discusses the relations between the groups.
     Results
     165cases of coronary group and65cases of control group, the plasma adiponectin, x±s respectively is6.26±2.01,9.39±2.56, two groups was statistically significant difference (P<0.05);
     265cases of coronary heart disease, stable angina with31patients (47.69%), plasma adiponectin8.98±2.56, unstable angina has24cases, accounting for36.92%of the plasma adiponectin6.02±2.03, myocardial infarction has10patients (15.38%), plasma adiponectin4.56±1.99, each two groups t test, P value were less than0.05, the difference was statistically significant;
     365cases of coronary heart disease, coronary artery disease a21cases, accounting for32.30%, plasma adiponectin7.98±2.16, two artery disease25patients (36.46%), plasma adiponectin5.02±1.98, three teams of coronary artery disease (19cases),29.23%, plasma adiponectin3.56±2.33, each two groups t test, P value were less than0.05, the difference was statistically significant;
     465cases of coronary group and65cases of control group, and baPWV value is measured respectively15.27±1.70m/s,12.34±2.50m/s, two independent sample mean more, P<0.05), and the value difference was statistically significant;
     565cases of coronary heart disease, stable angina with31cases, accounting for47.69%, baPWV value14.01±2.16, unstable angina has24cases, accounting for36.92%of the value baPWV15.02±1.03, myocardial infarction has10cases, accounting for15.38%, baPWV value15.56±1.39such as table3.3shows, the two groups each t test, P value are greater than0.05, the difference was not statistically significant.
     665cases of coronary heart disease, a coronary artery pathological changes of21cases, accounting for32.30%, baPWV value of13.78±2.26, two artery disease25cases, accounting for36.46%, baPWV value15.20±2.58, three teams of coronary artery disease (19cases),29.23%, baPWV value17.46±2.43, each two groups t test, P value were less than0.05, the difference was statistically significant;
     7Patients with coronary heart disease baPWV average determination value of15.27±1.70, we provisions in14.00measured value (m/s) the following as low baPWV group,14.00(m/s) or more as a high baPWV group, with lower baPWV group of36cases, high baPWV group of29cases, low baPWV group and high baPWV plasma adiponectin leels are two independent sample mean more, P<0.05), and the value difference was statistically significant.
     Conclusion
     1Plasma adiponectin, PWV are associated to the coronary heart disease, coronary heart disease patients plasma adiponectin levels lower than those of the control group, PWV value coronary artery group the higher than those in the control group;
     2Plasma adiponectin and coronary heart disease clinical types and levels are linked to the disease, coronary artery disease with the degree and range of aggravating, plasma adiponectin concentration also gradually reduced, there is a relationship between the coronary heart disease and PWV lesions degree, along with the pathological changes of the degree increased when the PWVD is increasing
     3When PWV of the patients with coronary artery disease is increasing, but the plasma adiponectin levels drop, there is a coordinated role.
