DICOM标准在放射治疗中的应用

摘要
DICOM标准是在医学信息学领域中正在兴起的标准。它定义了标准网络接口和数据模型使医学图像设备的制造厂商在标准网络上实现设备互联,简化了各种类型的医学图像的开发,推动开放式与厂牌无关的医疗数字影像的传输与交换, 促使影像归档及通讯系统PACS (Picture Archiving and Communication Systems) 的发展与各种医院信息系统HIS (Hospital Information Systems) 的结合。
     放射治疗俗称“照光”,就是利用放射线来杀灭肿瘤,达到治疗肿瘤的目的,是恶性肿瘤的主要治疗手段之一。随着科学技术的迅猛发展,出现了放射治疗计划系统,帮助物理师们更精确的给肿瘤定位并作出放疗计划,有效的杀死肿瘤。很多医院的放射治疗科购买了不只一个厂商的放疗计划系统,要使这些计划系统之间能够交流治疗信息必须使用DICOM标准来完成。
     DICOM在放射治疗中的应用称作DICOM RT。在国内,医生们还没充分意识到它的重要性,研究者们也没给予充分的重视;而在国外,此方面的研究已经如火如荼。
     本文主要探讨DICOM协议在放射治疗中的实现和应用。主要的工作包括:
    (1) 分析DICOM应用实体结构以及运行机制,实现了基于TCP/IP协议的DICOM网络通讯模型,完成了具有隐式表示低字节优先字节顺序、显示表示低字节优先字节顺序以及显示表示高字节优先字节顺序三种传输语法的服务对象对的传输。
    (2) 基于这个通讯模型实现了最基本的DICOM服务类:存储服务类。这个服务是指由一个DIMSE服务用户调用来请求另一个对等DIMSE服务用户存储复合SOP实例信息,它是最常用的DICOM服务类。
    (3) 解决了部分放射治疗计划系统DICOM数据接口问题,包括RT图像,RT结构集,RT 计划三个模块的实现,基本解决不同放射治疗计划系统间信息共享的问题。
DICOM is a standard developing in the field of medical informatics. It defines a standard network interface and data model. The employment of the standard realizes the connection of the different manufactories of medical imaging devices, simplifies the development of all kinds of medical images, facilitates the transmission and exchange of medical images and urge the combination of the PACS and HIS.
     The intent of radiotherapy is to make use of radiation to kill tumors. It's one of the methods to cure malicious tumors. With the development of science and technology, a radiotherapy planning system comes forth. It can assist physiatrists to accurately locate the tumors and design radiotherapy plans. Many hospitals buy the radiotherapy planning systems from not more than one manufactory. We should use DICOM to exchange information among the systems.
     DICOM RT is the abbreviation of DICOM in Radiotherapy. Inside the country, doctors have not recognized its signification, while outside the research is under way.
     This thesis discusses the realization of the DICOM RT. My work include:
    (1) Analyze the architecture of DICOM application entity and working mechanism and implement the model of DICOM network communication based on TCP/IP. Complete the communication between 3 kinds transfer syntax: Implicit VR Little Endian, Explicit VR Little Endian, Explicit VR Big Endian.
    (2) Based on this network communication model, realize the fundamental DICOM service class: C-STORE service class. The C-STORE service is used by a DIMSE-service-user to store a composite SOP Instance on a peer DIMSE-service-user. It is in common use.
    (3) Complete partial function of DICOM RT, including RT Image, RT Structure Set and RT Plan and implement the interoperability between two different RT planning systems
