冯新送教授学术经验总结及补肾活血方治疗膝骨关节炎的临床研究

摘要
目的
     为了传承和总结冯新送教授的学术思想、临床经验及诊治骨伤科疾病规律进行本课题研究。本课题是在他的学术思想的指导下,设计并实施补肾活血方治疗膝骨关节炎的临床研究。
     方法
     导师冯新送,男,广东省惠东县人,1944年8月出生,1969年毕业于广州中医学院医疗系本科。现任广州中医药大学教授、博士生导师、学位评审委员会副主任、广东省保健行业协会会长。大学毕业后留校,长期从事中医药教学、医疗、科研和行政管理工作。历任广州中医学院助教、讲师,南雄分院副院长,总务处副处长。广东省卫生厅中医处任副处长,兼广东省中医研究所副所长、所长,广东省第二中医院院长、广东一方制药厂董事长、广东省中医药局副局长。广州中医药大学任副校长、校长、教授、博士生导师、全国重点学科中医骨伤科学科带头人。
     冯教授擅长中医骨伤科,对中医骨伤科的临床有较深的造诣,尤其对骨关节炎、腰腿痛、老年性骨质疏松症、颈椎病、骨折与骨折病、颅脑损伤的中医治疗有独特疗效。他善于继承前人的经验,在老年骨伤科疾病的治疗中,学术上强调补肾壮骨、活血化瘀,对古方血府逐瘀汤、补阳还五汤有特别的研究,能灵活运用古方治疗骨科疾患,疗效显著。曾师从李家达、陈渭良、简华穆、魏正怀、朱通伯、李同生。冯教授培养的研究生26人。主编出版著作有9部,公开发表学术论文30篇。主持和参与科研项目9项,获奖项目1项。冯师学贯中西,学验俱丰,虽耳提面命3年,却难窥冯师学术全貌。本文从三方面简要总结冯师学术思想形成与医德风范。
     1、院校培养、学贯中西
     冯老师60年代毕业于广州中医学院(现为广州中医药大学)医疗系本科。求学期间,认真研读四大经典等中医经典原著,冯老师系统学习了现代西医学课程,经过5年系统学习,使冯老师“学贯中西”,成为即具有扎实的中医理论功底,又掌握西医诊疗技能和科研方法的新一代中医人才,为日后的临床医疗和科研工作打下了坚实的基础。
     2、加强继教、传承名医
     冯老师大学毕业后留校,长期从事中医药教学、医疗、科研和行政管理工作。虽然身兼行政职务,但每周尽量抽出时间出门诊和病房查房。
     他善于继承前人的经验,在中医骨伤科学术上强调活血化瘀、补肾壮骨,对古方血府逐瘀汤、补阳还五汤有特别的研究,能灵活运用古方治疗骨科疾患疗效显著。曾在佛山市中医院进修中医骨伤科,在广州市第一人民医院进修脑外科,在武汉医学院同济医院进修骨外科。1985年借调卫生部中医司,从事中医教育管理工作。在学术思想、治学方法和临证思维等方面深受各大名家的影响,使冯老师在老年骨伤科疾病治疗方面颇有建树,学有渊源。
     3、医者仁心,大医精诚
     冯老师经常教导学生“医者仁心,要因人治宜”,并身体力行地践行着这个座右铭。因此在治疗上要重视个体化、个性化治疗。在教学、科研和医疗工作中,冯老师始终要遵循“大医精诚”这四个字,并以它来严格要求自己。他的良好医德经常受到病人的好评。跟师三年来,通过跟师门诊,跟师查房及会诊,我对于老师临床疾病诊断,辨证思路,处方用药规律有了初步了解。本文从以下三个方面全面总结冯老师的整体学术思想与临床经验。
     1、衷中参西,病证结合,审因论治
     2、崇尚经典、善用经方,遣药精当
     3、调治气血、内外兼治、标本兼顾
     本文总结冯老师学术思想如下:
     冯老师关于“肾虚血瘀”病机理论的基本内容:冯老师认为中医老年骨伤科疾病大多是久病则机体受损,脏腑气血虚弱,其中以肾虚为其根本。老年人脏腑虚弱,气血运行无力,则瘀滞丛生,瘀滞成则怪病生。所以久病则瘀,诸虚皆可致瘀。冯老师提出“肾虚血瘀”学说思想,是从“虚”、“瘀”着手来研究揭示中医老年骨科疾病的一般病机规律。冯老师就是根据中医这些理论,结合自己长期的临床实践研究骨伤科疾病的发生、发展、转化及治疗方面的规律特点,总结归纳出中医骨伤科疾病“肾虚血瘀”病机理论这一规律。且根据这一理论指导临床治疗,取得了突破性进展,使中医骨伤科疾病治疗的疗效提高。本文分别从肾虚血瘀论治骨关节炎,骨质疏松症,腰腿痛几种临床常见骨科病来总结冯老师的临床经验。
     冯老师临床除了擅长治疗骨关节炎、骨质疏松症,腰腿痛等多种老年科骨关节退行性疾病,并积累了丰富的临床经验外。其他骨伤科疾病学术思想也非常丰富。本文总结了骨折与骨折病,骨科大手术后下肢深静脉栓塞治验,膝骨关节炎的中医手法治疗,中西医结合治疗跟骨粉碎性骨折等方面。
     在临床科研方面,设计并实施了补肾活血方治疗膝骨关节炎的临床研究,研究表明补肾活血方防治膝骨关节炎,效果满意,其机制可能是通过调控关节软骨细胞外基质的降解以及调节基质金属蛋白酶,抑制炎症,抑制氧自由基损伤,抑制软骨细胞凋亡,延缓软骨损伤,延缓关节退变,对骨关节炎的软骨有保护和促进修复作用。
     我的主要学习成绩
     按照国家中医药管理局及省中医药管理局的统一部署与要求,三年来,跟师门诊及查房学习270天,独立临床实践496天。收集典型病例60余例,完成跟师笔记181篇、月记36篇,经典学习体会11篇。发表导师临床经验论文2篇。参加了广州中医药大学师承博士班学习及考试。三年的师承学习即将结束,但学习没有完结,新一代的中医之路靠我们继续传承下去,所以我们今后的学习、工作任务十分繁重。我们也应把握机会,扛起重任,坚定信念,走中医之路,为弘扬祖国医学事业做出更大贡献。
     结论
     跟师学习近3年。在即将结业出徒之际,本文将冯新送教授学术思想渊源、相关各流派、学术经验及诊治骨伤科疾病规律研究做一总结。紧密结合临床,分类、归纳。
     在临床科研方面,设计并实施了补肾活血方治疗膝骨关节炎的临床研究,研究表明补肾活血方防治膝骨关节炎,效果满意,其机制可能是通过调控关节软骨细胞外基质的降解以及调节基质金属蛋白酶,抑制炎症,抑制氧自由基损伤,抑制软骨细胞凋亡,延缓软骨损伤,延缓关节退变,对骨关节炎的软骨有保护和促进修复作用。
Objective
     This study is carried out to inherit and summary the academic ideological and school, academic experience and the regular research on treatment. of orthopedic diseases of Professor Feng Xinsong. The study is designed with treatment of knee osteoarthritis with prescription of reinforcing kidney and activating blood in his academic thought guidance.
     Methods
     Professor Feng Xinsong, male, Huidong County, Guangdong Province, born in August1944, graduated from the Guangzhou College of Chinese Medicine in1969. He is Serving as professor, mentor, Dr., deputy director of degree evaluation committee of the Guangzhou University of Chinese Medicine, director of the health industry association of Guangdong Provincial. After graduation, he has long been engaged in medical teaching, medical, scientific and administrative management, served as Guangzhou Medical College, served as assistant, lecturer, vice president of Nanxiong points, deputy director general, deputy director of Institute of Traditional Chinese Medicine at the Guangdong Provincial Health Department, deputy director and director of Guangdong Provincial Institute of Traditional Chinese Medicine, Director of GuangDong Second Provincial Traditional Chinese Medicine Hospital, chairman of the Guangdong YiFang pharmaceutical, deputy director of Guangdong Bureau of Traditional Chinese Medicine, vice president, principals, professors mentor, Dr., leader of national key disciplines on orthopedics division of Guangzhou University of Chinese Medicine.
     Professor Feng have deeper knowledge on orthopedics of Chinese Medicine especially on the treatment of osteoarthritis, low back pain, osteoporosis, cervical spondylosis, fracture disease, traumatic brain injury with Chinese medicine. He is good at inheritancing from previous experience. He focused on reinforcing kidney and filling bone in the treatment of elderly orthopedic. He had special studies on the ancient prescription of Xuefuzhuyu and Buyanghuaiwu, which flexible used has made good effect on orthopedic disease. Professor Feng was under the tutelage of Li jiada, Chen wei Liang, jian Huamu, Wei zhenghuai, Zhu Tongbo, Li Tongsheng. Professor Feng trained26graduate students. There are nine published books edited, thirty published papers. He participated in nine research projects, an award-winning projects. Professor Feng had learning experience of Chinese and Modern medicine, which are difficult for me to grasp through three years' study. This paper briefly summarizes three aspects of academic theory and medical ethics.
     1. Training in college, Learning Chinese and Modern medicine
     Professor Feng graduated from Guangzhou Medical College (now Guangzhou University of Chinese Medicine) in the1960s. During the study, he made a careful learning on classics of Chinese Medicine such as the four original classic and a systematic study of modern medicine courses, which made him a new generation of Chinese Medicine with solid theoretical foundation of Chinese medicine and skills, research method of modern medicine, which layed a solid foundation for the future clinical and research work.
     2. Fnhancirig continuing education, Heritaging doctors
     Professor Feng stayed in the university after graduation, has long been engaged in eaching, medical, scientific and administrative management. He tried to take time out each week to ward and rounds. He is a good at inheritancing previous experi ence, laves academic emphasi s on activating blood, reinforcing kidney and filling bone. He makes it flexibility to treat the orthopedic disease with ancient prescription which makes a significant effect. He has been trained with orthopedic in Fosban Hospital of Chinese Medicine and Wuhan Tongji Hospital of Wuhan college,wit h neurosurgery in Guangzhou First People' s Hospital, He engaged in general practice education management in Chinese Ministry of Health in1985. lie was deeply impacted by various masters in academic thinking, scholarly approach and treatment thinking, which made many achievements on the treatment of orthopedic diseases in old age.
     3. the Benevolence, Large Health Sincere
     Professor Feng often teached students the "doctor benevolence, Concrete analysis of specific situations and set an example of the practice of this motto. Therefore, we should pay attention to the individual and personalized treatment.
     Professor Feng always followes the "Big Medicine sincere" in teaching, research and medical work, and is strict with himself with it. His good ethics are always praised by the patients. In this three years, I. had a preliminary understanding of disease diagnosis, dialectical thinking and the laws of prescription drug. In this paper, a comprehensive summary of Professor Feng will be explained with the following three aspects
     1. Comprehensive reference for Chinese Medicine and Modern Medicine. Combining disease with syndrome. According to etiology therapy.
     2. Advocating classical.Specializing in the use of Classical Prescriptions. Precising and appropriate drug removal.
     3. Modulating Qi and blood. Internal and external treatment. Both specimens.
     This paper summarizes Professor Feng academic thinking as follows:
     The basic content of the theory "kidney deficiency and blood stagnation ": Professor Feng found there are more elderly orthopedi c di seases which caused by the chronic body damage, weakness of Qi and blood, of which the kidney deficiency was the fundamental. The organs of aged are weak, Qi and blood run slowly, which make stasis and the abnormal health. So long illness leades to stasis, weakness can lead to stasis. Professor Feng put forward the theory " kidney deficiency and blood stagnation ", which can reveal the law of elderly orthopedic disease of Chinese Medicine from the weakness and stasis. He summarized the theory "kidney deficiency and blood stagnation" based on Chinese Medicine theories with his long-term study of clinical orthopedic diseases research on its development, transformation characteristics and treatment aspects. Guided with this theory in clinical therapy, he achieved a breakthrough which improved the efficacy of treatment of orthopedic disease, In this paper, the clinical experience of Professor Feng will be intrduced from kidney deficiency and blood stagnation on the treatment of osteoarthritis, osteoporosis, low back pain.
     Professor Feng accumulated a weal th of other academic thinking in addition to good clinical treatment of osteoarthritis, osteoporosis, low back pain and other degenerative bone and joint diseases. This paper summarized the fracture and fracture disease, lower extremity deep vein thrombosis treatment experience after major orthopedic surgery, Chinese Medicine practice on knee osteoarthritis, treatment of calcaneal fractures with Chinese Medicine and Modern Medicine.
     In clinical research, treatment of knee osteoarthri tis with prescription of reinforcing kidney and act ivat i ng blood was designed, which have shown that the treatment of knee osteoarthritis with prescription of reinforcing kidney and activating blood got satisfactory results. The mechanism may be work by regulating extracellular matrix degradation of articular carti1age which can regulate matrix metalloproteinases, inhibite inflammation and injury of oxygen free radical s, inhibite the chondrocyte apoptosis, delay the cartilage damage and joint degeneration, which can protect and repair the cartilage of osteoarthritis
     Mymainacademic
     In accordance with requirements of the State Pharmaceutical Administration of Chinese Medicine Administration and the unified deployment,1have rounded out with teacher for270days, finished independent clinical practice for496days, collected more than60cases of typical cases, completed181notes,36recorded monthly and11classic learning experience. I have published two papers of instructor' clinical experience, participated in1he study and examination organized by the Guangzhou University of Chinese Medicine. Three years of study in the end is coming. The learning is not the end. A new generation of Chinese medicine rely on our passing along. Our future study and work will be very heavy. We should seize the opportunity to carry the heavy, strong faith, take the road of Chinese medicine and make greater contributions to Chinese Medicine.
     Conclusion
     Learning with teacher for the past three years, this article summarise the law of Professor Feng Xinsong on academic ideological origins, related schools and academic experience of orthopedic diseases in the upcoming closure of the occasion.
