平面照相法对去眼袋术效果评价的研究

摘要
目的:采用计算机辅助照相测量法对去眼袋手术病人术前及术后6月的下睑表面解剖结构标记进行测量研究,同时对19-24岁成人的相关下睑的数据测量进行测量研究,作为进行评价时的参照。为眼袋整形效果提供可量化的数据评价系统参考,同时对睑袋整形手术效果进行评价。
     对象与方法:随即选取长沙地区就读的102名18岁~24岁正常年轻大学生名(男57名,女45名,平均年,21.6岁)为研究对象。同时选取本科室门诊手术室做睑袋整形成功随访的18例病人(男4名,女14名,平均年龄44.7岁),使用佳能数码相机拍摄标准数码相片,然后再计算机上使用Image-Pro-Plus7.0图像分析软件对相片进行处理分析,测量下睑表面解剖各线性及角度指标的值,包括睑裂宽度、下睑缘长度、睑袋槽长度、睑袋槽高度、睑裂倾斜度,同时计算出睑袋槽高/睑裂宽度。所得数据正常组采用独立样本t检验,手术组采用配对t检验。
     结果:所有指标均计算得出均值及标准差。19-24岁成人组中有睑袋组:内眦间距男性为33.8±4.2(mm)女性为33.4±5.2(mm);睑裂长度(右)男性为25.2±2.8(mm),女性为25.0士3.1(mm);睑裂长度(左)男性为24.7±2.8(mm),女性为25.0±3.4(mmm);下睑缘长度(右)男性为26.4±2.8(mm),女性为25.6±2.2(mm);下睑缘长度(左)男性为25.6±2.2(mmm)女性25.7±3.3为(mmm);睑袋槽长度(右)男性为24.2±3.0(mmm),女性为24.8±4.9(mm);睑袋槽长度(左)男性为25.6±4.1,女性为24.3±4.4;睑袋高度(右)男性为8.5±1.4(mm),女性为8.8±1.7(mm);睑袋高度(左)男性为7.9±1.3mm),女性为8.6±1.2(mm);睑裂倾斜度(右)男性为8.6±2.2(°),女性为9.0±3.6(°);睑裂倾斜度(左)男性为6.8±2.3(°)女性为8.7±4.0(°);睑带槽高/睑裂宽度(右)男性为33.5±2.4(%),女性为35.3士6.6(%);睑袋槽高/睑裂宽度(左)男性为32.2±4.8,女性为34.7±5.1。人群睑袋槽总发生率为26.5(%)。所有指标均计算出均值及标准差。手术组:睑袋整形手术术前、术后效果显著:密切指标下睑袋槽长、下睑袋槽高、睑袋槽高/睑裂宽度的值有明显变化,差异具有统计学意义,(P<0.05),而其他指标如内眦间距、睑裂宽度则没有明显变化,差异不具有统计学意义(P>0.05)。
     结论:本研究测量了19-24岁成人睑袋整形相关的下睑表面解剖各线性及角度指标的值,包括睑裂宽度、睑袋槽、睑裂倾斜度等,同时计算出睑袋槽高/睑裂宽度的正常值范围。建立了睑袋整形相关下睑表面解剖结构的正常数据库,并为睑袋整形术前、术后方案制定和评价提供了一个可量化的参考数据。采用本方法,证明睑袋手术有明显的良好的效果。
Objective:Around the lower eyelid blepharoplasty,we use computer assisted image measurement to measure and research the anatomy of the orbit in lower eyelid of adults aged19-24to provide a reference for the evaluation of Pouch plastic operation. At the same time, we use this method to evaluate the effect of blepharoplasty operation the patients had before and after6months
     Subjects and Methods:We select102young undergraduates age from18to24years old (male57, female45, average,21.6years) in the reseach,and18patients after blepharoplasty (male4, female14, mean age44.7years) from our outpatient operating room.We use Canon digital camera to take standard digital photos,and use the software Image-Pro-Plus6image analysis to analyse the photo to measure the he linear and angle index value of orbital anatomy, including the width of palpebral fissure length, lower eyelid blepharoplasty, slot length, slot height, lower eyelid palpebral fissure inclination.Than calculate the value of the head groove height/width of palpebral fissure. We process the data of normal group with independent sample t test, and operation group by paired t test.
     Results:All indexes were calculated the mean and standard deviation. The group who had lower eyelid in normal group:Inner canthal distance of male was33.8±4.2(mm) in female was33.4±5.2(mm); palpebral fissure length (right) male was25.2±2.8(mm), female was25.0±3.1(mm); palpebral fissure length (left) male was24.7±2.8(mm), female is25.0±3.4(mm); the lower eyelid length (right) male was26.4±2.8(mm), female was25.6±2.2(mm); the lower eyelid length (left) male was25.6±2.2(mm)25.7±3.3for women (mm); eyelid bag slot length (right) male was24.2±3.0(mm), female was24.8±4.9(mm); eyelid bag slot length (left) male was25.6±4.1,24.3±4.4women; eyelid height (right) male was8.5±1.4(mm), female was8.8±1.7(mm); lower eyelid height (left) male was7.9±1.3(mm), female was8.6±1.2(mm); inclination of palpebral fissure (right) male was8.6±2.2(°), female was9.0±3.6(°); inclination of palpebral fissure (left) male was6.8±2.3(°) female was8.7±4.0(°); eyelid with grooves/palpebral fissure width (right) male was33.5±2.4(%), female was35.3±6.6(%); lower eyelid groove height/width of palpebral fissure (left)32.2for men±4.8,34.7±5.1women. The crowd eyelid groove total incidence was26.5(%).All the indexes to calculate the mean and standard deviation. Meanwhile, the operation group:blepharoplasty surgery before and after operation effect:close index lower eyelid slot length, slot, lower eyelid blepharoplasty high slot/palpebral fissure width values have changed significantly, the difference was statistically significant (P<0.05), and other indicators, such as the inner canthal distance, palpebral fissure width did not change much, difference was not statistically significant (P>0.05).
     Conclusion:This study measured the blepharoplasty of of adults aged19-24related to the eye of the linear and angle index of local anatomy, including the palpebral fissure width, head groove, inclination of palpebral fissure, and calculated head groove height/width of palpebral fissure in the range of normal value. Established the blepharoplasty related eye the normal anatomy of the database structure, and provides a quantitative reference data for the scheme and formulate blepharoplasty of preoperative and postoperative evaluation. By adopting the method of operation, that is obviously good effect.
