温阳化饮、益气活血法防治充血性心力衰竭的实验研究

摘要
充血性心力衰竭(CHF)是各种心脏病的终末阶段,临床发生率高,病死率高,是严重危害人民健康的常见疾病。目前CHF的治疗目标已从单纯改善症状向提高患者长期生活质量和降低病死率方面转变,而仅凭西药治疗仍难实现这一目标。西药治疗常须联合应用多种药物,存在一些难以避免的毒、副作用,而中医药治疗具有多靶点、多途径和副作用相对较小的优势。
     本研究通过分析大量的中医古代文献及近年来中西医对CHF的研究进展,结合我们的临床经验,认为阳气虚衰,瘀血痰饮互结是CHF的主要病机。在此基础上提出运用温阳化饮、益气活血法治疗CHF,选用附子、人参、黄芪、丹参等十位药物组成强心胶囊并进行了实验研究。
     本研究采用腹腔注射阿霉素法复制了目前公认的Wistar大鼠CHF动物模型,将动物模型分为中药预防组、中药治疗组和西药对照组(卡托普利、倍他乐克联合应用)三组进行药效学研究,并设立正常对照组和模型组作为对照。结果表明以温阳化饮、益气活血法组成的强心胶囊具有以下作用:(1)可明显改善CHF大鼠的心脏收缩和舒张功能;(2)能显著降低CHF大鼠血浆内皮素、肿瘤坏死因子、心钠素、醛固酮、血管紧张素Ⅱ的水平;(3)能改善CHF大鼠心肌的病理变化和超微结构;(4)强心胶囊能明显抑制CHF大鼠心肌细胞凋亡及调控相关基因Fas、Bcl-2的表达。以上结果提示:强心胶囊是防治心衰的有效药物,其在调整CHF大鼠的神经内分泌水平,降低内皮素和肿瘤坏死因子水平方面同西药对照组作用相当,其调控细胞凋亡相关基因表达、减少心肌细胞凋亡及改善心室重构作用优于西药对照组。温阳化饮、益气活血法可以从多种途径对CHF进行有效防治,早期用药效果更佳。
Objective: To detect the main pathogenesis of CHF and find an effective medicine to prevent the development of CHF and decrease its fatality in herbs.
    Materials and methods: 75 Wistar rats were devided into 5 groups at radom,15 rats per group.They are normal group,model group,preventive treating group,treating group and contrasting group.The rat model of CHF was created by injection ADR in abdomen for 6 weeks, once a week.The preventive treating group and treating group were given Qiangxin Capsule(QXC) composed of ten herbs including of Ginseng,Prepared Aconite Root and so on.The contrasting group were gave Betaloc and Captopril.After giving drugs for 4 weeks(treating group and contracting group) and 6 weeks(preventive treating group ),all rats' heart function were measured by 8-leads mechanocardiography .Then the circulatory level of Ang II ,ANP,Ald,ET,TNF- a in blood of rats were studied.
    Results: QXC can improve heart function of CHF rats,it can also decrease the level of Ang II ,ANP,Ald,ET,TNF- a in blood.QXC can prevent rat heart cells from apoptosis through adjusting releated genes.Finally the damage in rats' heart were leveaited by QXC.
    Conclusion: Dificiency of yang-qi with blood stasis and phlegm is the main pathogenesis of CHF.Invigorating yang-qi, promoting blood circulation and resolving dampness are valid therpy to CHF.QXC is an effective drug in treating CHF.
