益气活血、泄热熄风法治疗出血性中风急性期的临床验证研究

摘要
目的:验证益气活血、泄热熄风法治疗出血性中风急性期的临床疗效,为临床提供疗效确切、使用方便、易于推广的中西医结合治疗方案。
     方法:在全国33家医疗单位开展益气活血、泄热熄风法治疗出血性中风的临床研究,采用3:1(治疗组3例,对照组1例,按患者入院顺序)的对照研究方法。治疗组采用益气活血、泄热熄风法方案,即在常规西医治疗基础上,加用具有益气活血、泄热熄风功效的中风醒脑方;对照组采用常规西医治疗。观察患者治疗前后神经功能缺损、血肿吸收率、病死率、中医症状体征严重程度积分等指标的改变,评价治疗7天、14±2天、21±3天、90±5天等时间点患者的情况,同时观察治疗过程中的不良反应。
     结果:1.神经功能缺损情况:治疗组在21±3天、90±5天时神经功能缺损改善情况优于对照组,差异有统计学意义(P<0.05)
     2.血肿吸收率:21±3天时治疗组血肿吸收率优于对照组,差异有统计学意义(P<0.05);
     3.病死率:治疗21±3天时治疗组和对照组病死率差异无统计学意义;90天时治疗组病死率低于对照组,差异有统计学意义(P<0.05);
     4.中医症状体征改变:治疗7天、14±2天、21±3天治疗组中医症状体征积分改善情况优于对照组,差异有统计学意义(P<0.05);
     5.二组均未出现严重不良反应。
     结论:1、益气活血、泄热熄风法治疗方案能改善出血性中风急性期患者神经功能缺损情况,在治疗14±2天、21±3天、90±5天时改善患者的神经功能缺损程度优于单纯西医治疗方案;
     2、益气活血、泄热熄风法治疗方案能促进出血性中风急性期患者血肿吸收率,在21±3天时优于单纯西医治疗方案;
     3、益气活血、泄热熄风法治疗方案能降低出血性中风急性期患者病死率,在90±5天时疗效优于单纯西医治疗方案;
     4、益气活血、泄热熄风法治疗方案较西医对照组在改善症状体征的严重程度积分及对生活的影响程度上优于西医治疗方案。
     5、通过此次大样本、多中心临床验证,认为益气活血、泄热熄风法治疗方案疗效确切,是一个安全有效,使用方便,易于临床规范化操作和适合城乡应用的最佳方案。
Objective:To evaluate the effect of "FUYUANXINGNAO" treatment in acute Cerebral hemorrhage.
     Objects and Methods:Devided acute cerebral hemorrhage into two groups by ratio of3:1(3cases to the "FUYUANXINGNAO" treatment group and1to contrast group). The "FUYUANXINGNAO" treatment group cured by usual western medicine treatment and traditional Chinese medicine ZHONGFENGXINGNAO prescription, the contrast group cured by usual western medicine treatment only. Observing the patient's fatality, hematoma absorption rate, integration of the influence to life in TCM, integration of the neurologic deficits, evaluating the effectiveness in7days,14days,21days and90days.
     Results:Neurologic impairment:the change of the neurologic deficits of the treatment group is superior to the contrast group in14,21days and90days, there is statistical significance in comparison of two groups (P<0.05)
     1. Hematoma absorption rate:the absorption rate of hematoma of the treatment group is superior to the contrast group, there is statistical significance in comparison of two groups (P<0.05)
     2. Change of the symptoms and physical signs of TCM:the symptoms and physical signs of TCM of the treatment group is superior to the contrast group in7days,14days and21days, there is statistical significance in comparison of two groups (P <0.05);
     3. Fatality:There is no statistical significance in fatality of two groups before21days, but there is statistical significance in fatality of two groups after90days' therapy (P<0.05). The fatality of the treatment group is lesser than the contrast group;
     4. There is no obvious untoward reaction during the treatment process both of the two groups.
     Conclusion:1. The "FUYUANXINGNAO" treatment can reduce the fatality of acute cerebral hemorrhage in90days.
     2. The "FUYUANXINGNAO" treatment can promote absorption of hematoma. in21days.
     3. The "FUYUANXINGNAO" treatment is superior to the usual western medicine treatment in improving the neurologic deficits in21days and90days.
     4. The "FUYUANXINGNAO" treatment is superior to the usual western medicine treatment in integration of symptoms, physical signs and integration of influence to life, especially in hemiplegic paralysis, mouth slanting, stuttering, numbness of limbs, dizziness, fatigue, headache, and so on.
