鼻内镜上颌窦开放术90例疗效分析
摘要
目的：探讨鼻内镜手术治疗良性上颌窦病变的手术方法、适应症、并发症和疗效，并对相关问题做简要综述。方法：以Messerklinger术式，行鼻内镜下上颌窦开放术（包括单纯中鼻甲成型术+上颌窦负压置换3例）90例（134侧），其中，上颌窦膜性窗或骨窗扩大开放术83例，中、下鼻道联合开窗术4例，中、下鼻道融合开窗术2例，窦口开放+上颌窦内侧壁部分切除术1例。所有病例术后定期随访均在1年以上。结果：90例中，治愈80例（88．9%），好转6例（6．7%），无效4例（4．4%）。随访至术后9月时鼻内镜检查，77例（85．5%）上颌窦口开放良好，9例（10%）窦口狭窄但功能尚可；术腔粘连5例（5．6%），窦口闭塞4例（4．5%），鼻泪管损伤1例，眶纸板损伤2例，无复视、脑脊液鼻漏、视力下降等严重手术并发症。结论：术前仔细阅读CT，正确的开窗术是提高手术总体疗效的重要环节；如无必要应尽量减少对窦口黏膜的损伤，并应减少不必要的开窗术以防止医源性鼻窦黏液再循环；合理的上颌窦口黏膜处理技术可以降低术后窦口的闭塞率；保持中鼻甲的正常位置和形态是减少中鼻道粘连的有效方法。
