姜参七味饮治疗胃溃疡的配伍研究

摘要
胃溃疡属中医辨证“胃痛”、“吐酸”等病范围。本病多发于20～55岁青壮年，以男性居多，大约占内科住院人数的0.8～3％。胃溃疡病复发率高，若防治不当亦可引起上消化道出血、胃穿孔、幽门梗阻及癌变等。然现代医学目前尚缺少行之有效的药物。因此，从中医治法配伍组方入手，对其进行深入研究，寻求有效的治疗方法，提高治疗效果，无疑具有重要的意义。
本课题旨在通过古今文献研究、动物实验研究，从方剂学角度，阐明姜参七味饮以温中健脾、行气止痛、收敛制酸法配伍组方治疗胃溃疡的科学合理性及各法间的配伍关系。
古今文献研究表明，胃溃疡病因病机多与饮食不节、劳倦内伤，导致脾胃虚弱(寒)，气血失调；或频繁的七情刺激，特别是忧思恼怒等，引起肝胃不和，土虚木侮，气机郁滞有关。在本病的辨证治疗上，多用温中健脾、行气止痛、收敛制酸等法治疗，每能取得良好效果。
在动物实验研究中，观察了全方、阳性药及各拆方组对胃溃疡模型大鼠形成溃疡程度、溃疡指数、溃疡抑制率的影响，以及对大鼠胃液分泌的影响(以胃液分泌量、胃液酸度测定、胃蛋白酶活性及排出量测定为指标)。其结果表明：姜参七味饮以温中健脾，行气制酸法组方能显著改善胃溃疡模型大鼠胃粘膜的损伤病变，作用优于阳性对照药香砂养胃丸。对模型大鼠胃液分泌量、胃液酸度、胃蛋白酶活性等指标的改善，全方组作用明显。各拆方实验结果表明，不同治法相配伍时，其作用并非简单相加，而是具有特殊的协同作用。
Gastric ulcer belongs to Stomachic Pain, Acid Regurgitation, etc. in TCM. The incidence of this disease is about 0.8-3 percent of the medical patient, mainly among young and middle-aged people. Without promptly treatments or with erroneous treatments, Gastric ulcer may incur many complication such as gastric perforation, hemorrhage, pyloric stenosis, etc. Therefore, the further study of the disease, the quest of the effective therapy, the raise of the curative effect, certainly has great social significance and practical significance of study of TCM.
This subject aims at explicating in the angle of recipe course, about the rationality of the shen Jiang Qi wei Yin according to the therapeutic methods of warming the middle-warmer, invigorating spleen, circulation Qi, and restraining acid in treating gastric ulcer. The document research shows that, the pathogenesis of gastric ulcer mostly relates to injuries for improper foods or drink, emotional disturbances, fatigue, and so on. All sorts of reasons due to the decline of splenic functions together with the cold syndrome. In TCM therapeutics, warming the middle-warmer, invigorating
spleen, circulation Qi restraining acid, regularly proves effective.
During the experiment, we observed the model rat treated with different components. The result shows that, the prescription composed according to the therapeutic methods of warming the middle-warmer, invigorating spleen, circulation Qi, and restraining acid can distinctly ameliorate the gastric ulcer of the model rat. Different therapeutic methods, when making up with one another, have particular inner link.
本课题旨在通过古今文献研究、动物实验研究，从方剂学角度，阐明姜参七味饮以温中健脾、行气止痛、收敛制酸法配伍组方治疗胃溃疡的科学合理性及各法间的配伍关系。
