艾灸足三里、梁门穴对应激性溃疡大鼠胃粘膜细胞增殖、凋亡及HSP70的影响

摘要
目的:以胃粘膜损伤指数(UI)为评定指标,观察艾灸大鼠足三里、梁门穴对胃粘膜损伤的保护作用。观察艾灸对应激性溃疡大鼠胃粘膜细胞增殖与凋亡的影响,检测血浆多巴胺(DA)和胃粘膜转化生长因子α(TGF-α)、前列腺素E2(PGE2)的含量,分析热休克蛋白70(HSP70)与上述因子的关系,探讨艾灸促进胃粘膜损伤修复的机制。
     方法:60只健康SD大鼠随机分为4组,即空白组、模型组、艾灸足三里、梁门穴组和艾灸非穴对照点组。束缚水浸应激法制备胃溃疡模型,采用高效液相色谱法测定血浆DA的含量,放射免疫方法测定胃粘膜TGF-α和PGE2的含量,免疫组织化学方法检测胃粘膜HSP70的表达及胃粘膜细胞增殖指数(PCNA-LI)和凋亡指数(AI)。
     结果:1、与正常组比较,束缚水浸应激法造模后,模型组胃粘膜损伤指数升高(P<0.01);艾灸足三里、梁门穴组胃粘膜损伤指数显著降低,与模型组和非穴组比较有显著性差异(P<0.01),与空白组比较则无显著性差异。
     2、模型组血浆DA含量(ng/mL)显著高于空白组(P<0.01);艾灸足三里、梁门穴组和非穴组血浆DA含量较模型组都显著降低(P<0.01),与空白组比较无显著性差异,且足三里、梁门穴组效果优于非穴组。
     3、模型组胃粘膜TGF-α含量(pg/mL)显著低于空白组(P<0.01);艾灸足三里、梁门穴组胃粘膜TGF-α含量显著升高,与模型组比较有显著性差异(P<0.01);非穴组胃粘膜TGF-α含量也升高,与模型组比较无显著性差异。
     4、模型组胃粘膜PGE2含量(pg/mL)显著低于空白组(P<0.01);艾灸足三里、梁门穴组胃粘膜PGE2含量显著升高,与模型组和非穴组比较有显著性差异(P<0.01),非穴组与模型组比较无显著性差异。
     5、模型组胃粘膜HSP70表达显著高于空白组(P<0.05);艾灸足三里、梁门穴组胃粘膜HSP70表达显著高于其余各组(P<0.05或P<0.01),非穴组胃粘膜HSP70表达与模型组比较无显著性差异。
     6、模型组胃粘膜细胞PCNA-LI显著低于空白组(P<0.05);艾灸足三里、梁门穴组与非穴组的胃粘膜PCNA-LI均显著高于模型组(P<0.01或P<0.05),且艾灸足三里、梁门穴组胃粘膜细胞增殖较非穴组明显。
     7、模型组胃粘膜细胞AI显著高于空白组(P<0.05);艾灸足三里、梁门穴组胃粘膜AI显著低于模型组和非穴组(P<0.05),非穴组胃粘膜AI与模型组比较无显著性差异。
     结论:1、艾灸足三里、梁门穴对应激性溃疡胃粘膜具有保护作用。2、其机制可能是调节血浆DA含量对抗应激性溃疡,促进胃粘膜TGF-α和PGE2的合成、加速胃粘膜修复,刺激胃粘膜细胞增殖、抑制细胞凋亡。3、这一过程与艾灸诱导HSP70表达有关。4、艾灸足三里、梁门穴保护胃粘膜具有穴位相对特异性。
Objective:To observe the protective effects of moxibustion at "Zusanli" and "Liangmen" on gastric mocosa of stress ulcer rats according to ulcer index. To observe the effects of moxibustion on proliferation and apoptosis of gastric mucosal cells and the content of dopamine (DA) in plasma, Transforming Growth Factor a(TGF-α) and prostaglandin E2 (PGE2)in gastric mucosa, explore the relationship between the effects and Heat Shock Protein 70 (HSP70), and the mechanism of moxibustion to accelerate the repair of gastric mucosal lesion.
     Methods:Sixty healthy SD rats were randomly assigned to blank, model, moxibustion at "Zusanli" and "Liangmen" and moxibustion at control points groups. Preparing gastric ulcer model by water immersion restraint, assaying the content of TGF-a and PGE2 in gastric mucosa by radioimmunoassay and DA in plasma by high efficiency liquid chromatography, observing the expression of HSP70 and Proliferating Cell Nuclear anti-gen (PCNA), and cell apoptosis index by immunohistochemical method.
     Results:1. Compared with model group, UI of model group increased (P<0.01); UI of moxibustion at "Zusanli"and "Liangmen"group decreased significantly (P<0.01) compared with model and control points group, and no significant difference with blank group.
     2. The content of DA in plasma of model group was higher than that in blank group significantly (P<0.01); that of moxibustion at "Zusanli" and "Liangmen " group and control points group decreased significantly compared with model group, and no significant difference with blank group. The content of DA in plasma of moxibustion at "Zusanli" and "Liangmen" group was closer to that of blank group compared with that of control points group.
     3. The content of PGE2 in gastric mucosa of model group was significantly lower than that of blank group (P<0.01); and that of moxibustion at "Zusanli" and "Liangmen" increased significantly compared with model group and control group (P<0.01). But no significant difference between model group and control points group.
     4. The content of TGF-αin gastric mucosa of model group was significantly lower than that of blank group (P<0.01); and that of moxibustion at "Zusanli" and "Liangmen" group increased significantly compared with model group; that of control group also increased but no significant difference with model group.
     5. The expression of HSP70 in gastric mucosa of model group was higher than that of blank group significantly (P<0.05); that of moxibustion at "Zusanli" and "Liangmen" group was higher than those in other groups (P<0.01 or P<0.05). There was no significant difference between control points group and model group.
     6. PCNA-LI in gastric mucosa cells of model group was lower than that of blank group significantly (P<0.05); PCNA-LI of moxibustion at "Zusanli" and "Liangmen" group and control points group were higher than that in model group (P<0.01 or P<0.05). There PCNA-LI of moxibustion at "Zusanli" and "Liangmen" group was higher than that of control points group.
     7. AI in gastric mucosa cells of model group was higher than blank group (P<0.05); AI of moxibustion at "Zusanli" and "Liangmen" group was lower than that in model group and control points group (P<0.05). There was no significant difference between control points group and model group.
     Conclusion:1. Moxibustion at "Zusanli" and "Liangmen" has the protective effects on gastric mucosa of stress ulcer rats.2. The mechanism may be regulating the content of DA in plasma to antagonize the ulcer, promoting the synthesis of TGF-a and PGE2 in gastric mucosa to accelerate the repair of gastric mucosa, accelerating the proliferation of gastric mucosa cells and suppressing the cell apoptosis.3. The process was related to the expression of HSP70 induced by moxibustion.4. Moxibustion at "Zusanli" and "Liangmen" to protective gastric mucosa was distinctive.
