“火郁发之”法对胃粘膜急性损伤中攻击因子与保护因子调节作用的研究

摘要
目的:胃粘膜是胃体的重要保护屏障,由胃粘膜损伤而引发的一系列消化道疾病,发病率高,且复发率亦高。因此,如何保护和修复受损胃粘膜,从根本上杜绝胃炎、胃溃疡等病的发生和康复,是医学领域的重要课题。
    在致病因素的作用下攻击因子和保护因子失衡,是造成胃粘膜损伤,引发各种急慢性胃炎及溃疡病等的首要环节。因而研究相关细胞因子的平衡关系,对阐明中药对胃粘膜保护的机理有重要意义。中医将胃粘膜损伤性疾病分属于胃脘痛、痞证、吞酸、呕吐及呃逆等病。在这些病症中,以火郁证居多。因而本课题选择了体现火郁发之治则中,辛开苦降法的胃康胶囊为施加因素,用消炎痛造成急性胃粘膜损伤的病理模型,研究对胃粘膜攻击因子、保护因子的调节作用,以阐明中医药治疗胃粘膜损伤的机理。
    方法:将50只Wistar大鼠雌雄各半,随机分成5组,即空白组10只,模型组10只,丽珠得乐组10只,胃康高剂量组10只,胃康胶囊低剂量组10只,空白组常规饲养,不作任何处理。将其余4组动物灌胃给药,模型组给2ml生理盐水;丽珠得乐组给2ml0.6%丽珠得乐悬液;胃康胶囊高剂量组给2ml32%胃康胶囊悬液;胃康低剂量组给2ml8%胃康胶囊悬液。每日1次,连续5天,于给药第4天禁食24小时,禁水4小时。末次给药1小时后1ml0.5%消炎痛溶液灌胃。测定大鼠血浆的TXB2、PAF、TNF-α、PGI2、EGF及胃粘膜中MDA和SOD的含量,数据用统计学处理。并观察胃粘膜病理变化。
    结果:胃康胶囊能够降低实验性胃粘膜急性损伤大鼠血浆的PAF、TNF-α和MDA含量,升高EGF、PGI2和SOD的含量,对抗胃酸分泌,减轻胃粘膜损伤的程度,对防止胃粘膜损伤的发生有肯定意义。
    结论:胃康胶囊可通过削弱攻击因子,增强保护因子2个环节发挥细胞保护作用,从而保护胃粘膜免遭损伤。
As a kind of highly easy recurrent diseases of multiple causes, the main disease mechanism of gastric mucosal damage chiefly lies in the mutual disorder of aggressive factor and protective factor caused by the regulation abnormality of neurohumor. The frequent recurrence of the disease can greatly affect the patients' living quality and 1ife security. Medicines with the function of reduceing gastric acid, rooting out the HP infection and strengthening the gastric mucosa protection are always abopted by modern medicine, which proved unsatisfactory result and no superiority in anti-recurrence. As the creation of prof. Tian Shuxiao through more than thirty years of clinical experience, Weikang capsule is a compound Chinese herbal product with safe efiect and lower recurrence rate in treating gastric mucosal damage diseases in clinic. This paper made a reserch on the effect of relevant cytokine involved the onset; development and healing of gastric mucosal damage diseases.
    We made the acute gastric mucosal damage with 40 Wistar rats and made them take Weikang capsule. And compared with the vacancy group the results suggested that the capsule could reduce the PAF and TNF-α content in blood plasma cytokine of experimental gastric mucosal damage rats and increase content of EGF and PGI2, inhibit the increase of gastric acid secretion, decrease the recurrence rate of gastric mucosal damage. The capsule also can alleviate uleury degree of gastric mucosa and prevent the formation of gastric mucosal damage. Pathological picture analysis showed that the capsule could treat gastric mucosal damage effectively and hasten the healing of the damaged gastric mucosal.
    We a1so made another experiment of the acute model with 40 wistar rats to observe the effect of Wiekang capsule in adjusting content of MDA and SOD in gastric mucosal. The result showed the capsule could depress the damage of LPO to prevent the onset of gastric mucosal damage effectively.
    In short,Weikang capsule could prevent the onset of gastric mucosal damage by weakening aggressive factors and strengthening protective factors.
