丁香胃灵合剂对慢性萎缩性胃炎大鼠Bcl-2、IL-2含量的影响

摘要
研究背景及目的:慢性萎缩性胃炎(CAG)是以胃粘膜发生腺体萎缩性改变的慢性炎症,CAG在胃粘膜癌前病变中占有重要地位,经过多阶段、多基因的变异积累过程可发展成胃癌。CAG是消化系统常见疾病,其发病机制尚不十分清楚,近年来逐渐成为医学界研究的热点,随着对CAG的深入研究,国内外学者提出CAG的发展变化是胃癌演变过程中的关键环节,已被明确定为癌前病变。本病由于病期长且反复发作,现代医学尚缺乏理想的特异性治疗方法,近年来多从中医领域采用中医理论治疗CAG,大量报道中医药对CAG病理改变有明显的逆转作用,可防治癌前病变发生。本课题通过研究丁香胃灵合剂对慢性萎缩性胃炎大鼠Bcl-2蛋白表达、IL-2含量的影响及胃粘膜病理组织学变化的观察,深入研究其作用机理。
     材料和方法:将Wistar大鼠60只(雌雄各半)随机分为正常对照组、模型对照组,丁香胃灵合剂低、中、高剂量组等5组,每组12只,雌雄各半。造模方法:正常对照组大鼠给予生理盐水灌胃,其余各组均采用水杨酸钠灌胃,自由饮用氨水,配合饥饱失常法建立CAG动物实验模型,造模时间2个月,造模成功后进行干预实验,丁香胃灵合剂各治疗组给予相应剂量的丁香胃灵合剂灌胃治疗6周。造模成功后处死全部实验大鼠,采用摘取眼球法取血,分离出血清,采用IL-2 Elisa试剂盒检测IL-2含量;剖胃取材,用PH试纸测定胃液PH值,光镜下观察胃粘膜病理组织学改变,免疫组化方法检测大鼠胃粘膜Bcl-2蛋白的表达。所有实验数据应用SPSS16.0 for windows统计软件进行统计分析,取P<0.05为有统计学意义,P<0.01为有显著意义。
     结果:模型对照组胃液PH值、IL-2含量、Bcl-2阳性表达指数明显高于正常对照组,差异有显著意义(P<0.01),病理组织学也有很大改变;丁香胃灵合剂各治疗组胃液PH值、IL-2含量、Bcl-2阳性表达指数明显下降,与模型对照组差异有显著意义(P<0.01),胃粘膜病理组织学明显改善。
     结论:IL-2介导的炎症和免疫反应以及Bcl-2的抑制细胞凋亡作用是CAG发病机制之一。丁香胃灵合剂对IL-2含量和Bcl-2蛋白表达水平具有调节作用,可抑制CAG大鼠胃粘膜细胞的过度增殖,可逆转CAG胃粘膜的病理改变,这可能是丁香胃灵合剂有效治疗CAG的作用机制之一,可进一步对丁香胃灵合剂治疗CAG的作用机制采用现代医学的方法进行研究。
Study Background and Objective:Chronic atrophic gastritis is a common disease of digestive system, It is a hotspot of digest field in recent years.CAG is gastric gland atrophy as the main performance inherent in the chronic inflammation, CAG in the gastric lesions precancerous occupy an important position, their movement through the multi-stage, multi-gene mutation accumulation process can be developed into the stomach. Its pathogenesis is not yet clear, with the in-depth study of the CAG, CAG foreign scholars put forward the development and changes in gastric cancer in the evolution of key links have been has been clearly identified as precancerous lesions. Due to the long course of the disease, Western medicine is still a lack of specificity of an ideal treatment method, etc. In recent years, the area from the use of traditional Chinese medicine theory, Chinese medicine treatment of CAG, a large number of reports that Chinese medicine has a clear reversal of the role of CAG, This study to observe the effect of Clove wei ling Mixture on the gastric mucosa level of IL-2、Bcl-2 and gastric mucosa Pathological histology in rats model of Chronic Atrophic Gastritis, and to explore the mechanism of Clove wei ling Mixture in the treatment of CAG.
     Material and Method:Sixty Wistar rats were randomly divided into Five groups (male and female half and half):normal control group, model control group, Clove wei ling Mixture low-dose group, Clove wei ling mixture middle-dose group and Clove wei ling mixture high-dose Group,12 in each group. Modeling methods:the normal control rats given normal saline, the other groups were administered with sodium salicylate, free drink ammonia, with irregular diet animal model was established by CAG, all the factors were given for two months. after the success of the intervention modeling experiment, Except normal control group rats and model control group, others were treated with different dose clove wei ling mixture for six weeks. Their gastric juice was collect to detect PH value with PH test paper, their blood serum of eyeball was collect to detect IL-2 with IL-2 Elisa kit IL-2 content; their stomachs were collect to detect Bcl-2 by immunohistochemistry to detect Bcl-2 in gastric mucosal expression. detect the gastric mucosa changes by light microscopy. All experimental data application SPSS 16.0 for windows statistical software for statistical analysis, P<0.05 as statistically significant, P<0.01 as statistically significant.
     Results:Compared to normal control group, PH value of gastric juice、the quantity of IL-2 and expression of Bcl-2 in the model control group was significantly higher, the difference was statistically significant (p<0.01),and pathological changes are quite; Compared to model control group, those rats were treated with different dose Clove wei ling mixture for six weeks, there was significant statistics difference in pathologic changes of gastric mucosa. Compared to model control group, PH value of gastric juice、the quantity of IL-2 and expression of Bcl-2 of high-dose Clove wei ling mixture group, middle-dose Clove wei ling mixture group and low-dose Clove wei ling mixture group was lower, the difference was statistically significant (p<0.01).
     Conclusion:IL-2 mediated inflammatory and immune response and inhibition of Bcl-2 Apoptosis is the pathogenesis of CAG. Clove wei ling mixture can adjust significantly level of the quantity of IL-2 and expression of Bcl-2. Protection of gastric mucosa injure is a important factor induced CAG, And Clove wei ling mixture have protective functions against the gastric mucosal injure, even can turn over partly the pathologic changes of gastric mucosa, It is a important factor induced Chronic atrophic gastritis
