SLE患者外周血单个核细胞雌激素受体、IL-10和IFN-γmRNA表达异常的研究及其相关性分析

摘要
目的:
     检测系统性红斑狼疮患者未经刺激外周血单个核细胞雌激素受体,IL-10和IFN-γmRNA的表达水平并分析其相关性,探讨其在狼疮发病中的作用。
     方法:
     分离25例系统性红斑狼疮患者和15例正常人新鲜外周血单个核细胞,提取总RNA,利用半定量RT-PCR方法逆转录并扩增雌激素受体,IL-10和IFN-γ,通过凝胶图像扫描系统对PCR产物的电泳条带进行密度扫描。
     结果:
     1 病例组PBMCsER和IL-10mRNA表达水平高于正常对照组(P<0.01)。SLE肾炎组和非肾炎组ER和IL 10mRNA表达水平均比对照组高(P<0.05)。SLE肾炎组均稍高于非肾炎组,但差异没有统计学意义。
     2 病例组PBMCsIFN-γmRNA表达水平高于正常对照组(P<0.05)。SLE肾炎组IFN-γmRNA表达水平高于非肾炎组和正常对照组(P<0.01)。SLE非肾炎组和正常对照组没有明显差别。
     3 病例组PBMCsER(r=0.176,P>0.05),IL-10(r=0.197,P>0.05)和IFN-γ(r=0.172,P>0.05)mRNA表达水平与患者疾病活动指数均不相关。
     4 病例组r=0.442(P<0.05)和对照组r=0.557(P<0.05)PBMCsERmRNA表达水平与IL-10mRNA表达水平均呈正相关。

    结论:
     SLE患者外周血单个核细胞存在雌激素受体,IL一10和IFN一YmRNA表达
    异常,IL一10表达的上调可能与雌激素受体有关,这可能是性激素影响细胞因
    子的作用机制之一。过量的IL一10和IFN一Y可能导致了SLE患者B细胞产生
    大量自身抗体。IFN一Y产生的增高可能触发了炎症反应的发生,在狼疮肾炎发
    生过程中起重要作用。
Objective:
    To examine the mRNA expression of estrogen receptor, IL10 and IFN-y in unstimulated peripheral blood mononuclear cells (PBMCs) from patients with systemic lupus erythematosus (SLE) and analyze their correlations, to investigate the role of estrogen receptor, IL10 and IFN-y in the pathogenesis of SLE.
    Methods:
    Isolating the fresh PBMCs from 25 women with SLE and 15 healthy women and extracting the total RNA followly. Then the mRNA expression of estrogen receptor, IL10 and IFN-y were measured semi quantitatively by reverse transcriptase-polymerase chain reaction (RT-PCR).
    Results:
    1. The mRNA expression of estrogen receptor and IL-10 in PBMCs of patients with SLE was significantly higher than that of healthy controls (P<0.01). There was no defference of the mRNA expression of estrogen receptor and IL-10 in PBMCs between SLE patients with nephritis and SLE patients without nephritis, though the former was higher than the latter, and both of them were higher than healthy controls (P<0.05).
    2. The mRNA expression of IFN-y in PBMCs of patients with SLE was higher than that of healthy controls (.P<0.05) and that of SLE patients with nephritis was
    
    
    higher than that of SLE patients without nephritis (P<0.01), There was no defference of the mRNA expression of IFN-y in PBMCs between SLE patients without nephritis and healthy controls.
    3. There was no correlation between the mRNA expression of estrogen receptor and SLEDAI in patients with SLE, neither between IL-10 and SLEDAI, IFN-Y and SLEDAI.
    4. There was positive correlation between the mRNA expression of estrogen receptor and IL-10 in patients with SLE (r=0.442, P<0.05) or healthy controls (r=0.557, />0.05).
    5. There was no correlation between the mRNA expression of estrogen receptor and IFN-y in patients with SLE or healthy controls.
    6. There was no correlation between the mRNA expression of estrogen receptor and the serum IgG in patients with SLE, nether between IL-10 and IgG.
    Conclusion:
    Our present findings suggest that there were mRNA expression of estrogen receptor, IL-10 and IFN-y abnormalities in patients with SLE. The expression of IL-10 maybe up-regulate by estrogen receptor and maybe that was one of the mechanisms how hormone affect the production of cytokines. Excessive IL-10 and IFN-y maybe resulte in autoantibody production by B cells in human SLE. The enhanced production of IFN-y by mononuclear cells may trigger inflammatory responses involving in the pathogenesis of SLE nephritis.
