桃仁红花煎治疗冠心病室性早搏疗效观察及延胡索抗心律失常实验研究

摘要
目的:观察桃仁红花煎治疗冠心病室性早搏的临床疗效并对方中主药延胡索抗心律失常作用机理进行探讨。
     方法:临床观察60例冠心病频发室性早搏患者,采用随机对照方法分成两组,治疗组与对照组各30例。对照组给予常规西药治疗,治疗组在常规西药治疗基础上加用桃仁红花煎,观察治疗前后临床症状及早搏疗效变化。将60只SD大鼠随机分成空白对照组、经典预处理组、假手术组及延胡索碱高、中、低剂量组6组,观察不同处理办法及药物不同剂量对大鼠心肌缺血再灌注损伤的影响。
     结果:经治疗2周后,试验组早搏总疗效的有效率与对照组相比并未增加,但中医证候疗效与对照组相比有显著性差异(P<0.05),某些中医症状疗效与对照组相比有显著性差异(P<0.05),实验研究显示延胡索可减少心肌缺血再灌注损伤模型大鼠的心律失常发生率,并能抑制损伤心肌细胞的凋亡。
     结论:桃仁红花煎治疗冠心病室性早搏能改善中医证候的疗效,延胡索可减少心肌缺血再灌注损伤模型大鼠的心律失常发生率,机理与抗心肌凋亡有一定关系。
Objective: To observe the clinical effect of TaoRenHongHuaJian andYanhusuo alkaloid on ventricular premature systole of coronary heartdisease and explore its mechanism of effect.
     Methods: Sixty patients of ventricular premature systole were dividedrandomly into treatment group(30 cases) and control group(30 cases).Conventional medical therapy was given to the both groups and thetreatment group received additional Chinese medicinal formulaeTaorenhonghuajian treatment. The changes of clinical symptoms andtherapeutic effect of premature systole in all the patients were observedbefore and after treatment. Sixty SD rats were divided randomly into blankcontrol group, sham operation group, classics fore treatment group andgroup of myocardial ischemia reperfusion model with Yanhusuo alkaloidhigh, moderate, minimal dose. To observe the impact of differentmanipulate ways and all the different dosage on the rats.
     Results: In the treatment group, the total effective rate of melioratingsyndrome and some symptoms of TCM were significantly better than thoseof the control group(P<0.05) while therapeutic effect of prematuresystole did not show the difference after two weeks' treatment, animaltest show Yanhusuo alkaloid can decrease the rate of arrhythmia in ratmodel with myocardial ischemia reperfusion and reveal Yanhusuo alkaloidhave some protection to myocardial damage.
     Conclusion: TaoRenHongHuaJian can improve syndrome of TCM on treatingventricular premature systole of coronary heart disease, Yanhusuoalkaloid can decrease the rate of arrhythmia in rat model with myocardialischemia reperfusion due to some protection to myocardial damage.
