犬急性胰腺炎病理模型的构建及临床治疗研究

摘要
[目的]探讨犬急性胰腺炎病理模型的构建以及清开灵注射液对犬急性胰腺炎的治疗作用。
     [方法]实验选用20只本地健康杂交犬作为研究对象,实验分为5组,每组4只实验犬,其中A组为空白对照组,B组为AP(急性胰腺炎)组,C组为清开灵治疗Ⅰ组,D组为清开灵治疗Ⅱ组(2倍剂量治疗组),E组为常规治疗组。构建急性胰腺炎模型采用胰管逆行注射法即在主胰管注入牛磺胆酸钠与胰蛋白酶复合液。模型构建成功后每天按各组规定药物治疗一次,共7天7次;且于构建模型后1,2,4,7天采集静脉血,动态地监测清开灵注射液对急性胰腺炎后犬血浆内毒素(ET),丙二醛(MDA),超氧化物歧化酶(SOD)以及淀粉酶(AMY)等含量变化。7天后采集内脏标本做细菌培养,观察细菌移位情况以及病理变化,探讨清开灵对急性胰腺炎的自由基以及内毒素清除情况和量效关系。
     [结果]在向主胰管内注入牛磺胆酸钠与胰蛋白酶复合液5-10分钟可看到胰腺充血出血严重,模型构建完成后各模型组血浆淀粉酶在第1天与空白对照组差异极显著(P<0.01),达到正常水平十倍以上,模型构建稳定。急性胰腺炎一旦发生,血液中的内毒素和丙二醛含量将随着时间的延长而升高,内毒素引起机体体温升高,而清开灵能显著降低机体的体温。常规治疗对内毒素和自由基的清除效果以及抑制细菌移位均小于清开灵治疗组(P<0.05);两个清开灵治疗组之间无明显差异(P>0.05)。AP组与3个治疗组之间在内毒素与自由基的清除和抑制细菌移位方面差异显著(P<0.05)。
     [结论]采用主胰管内注入牛磺胆酸钠与胰蛋白酶复合液可以构建稳定的急性胰腺炎病理模型;清开灵能显著清除急性胰腺炎时产生的内毒素和自由基,减少肠道细菌移位,减轻急性胰腺炎时胰腺的病理变化,快速降低体温,是临床上可以推荐使用的治疗犬急性胰腺炎的一种药物。
[Objective] Discussing about construction of canine acute pancreatitis pathology model and treatment with "qingkailing" .
     [Methods] A total of 20 dogs were randomly assigned to 5 treatment groups which were charge group, AP group, "qingkailing" group 1, "qingkailing" group 2 and regular treatment group. The cases modeling was building by antidrom-injecting compounds trypsin and Sodium Cholate into canals of wirsung. Then every dog of each group was treated 1 time by regular medicine every day. Every dog was sampled blood from jugular vein on 1~(st) day, 2~(nd) day, 4~(th) day and 7~(th) day after the case model was building in order to monitoring the variation of ET, MDA, SOD and AMY. And 7 days later, specimen from splanchna were sampled for bacterial culture to study the bacterial translocation and pathology. In order to approach the dose-effect relationship of radical scavenging and ET scavenging to "qingkailing" .
     [Results] Severe hyperaemia and hydropsia of pancreas could be seen after injecting sodium cholate. The difference of Plasma amylase between charge group and other groups was extremely significant. ET and MDA were increased with time after acute pancreatitis happened. Body temperature was aroused higher by ET while "qingkailing" made it lower. The diffirence effect of radical scavenging and ET scavenging and bacterial translocation was significant. There' s no significant difference between "qingkailing" group 1 and "qingkailing" group 2. the difference between AP group and other 3 treatment group.
     [conclusion] Case model of acute pancreatitis could be builded by injecting sodium cholate and compounds trypsin. ET and free radicle could be scavenged, translocated bacterial could be reduced pathological changes could be relieved and body temperature could be lower significant by injecting "qingkailing" . "qingkailing" is a usuful medicine on clinical.
