五倍子瘢痕膏治疗瘢痕疙瘩的疗效观察及其对成纤维细胞功能的调节作用

摘要
目的:观察五倍子瘢痕膏治疗瘢痕疙瘩的临床疗效及其对成纤维细胞增殖、凋亡和Ⅰ、Ⅲ型前胶原基因表达的影响。
     方法:临床疗效观察选择瘢痕疙瘩病人39例共82块瘢痕疙瘩,随机分为3个组,分别予以五倍子瘢痕膏、黑布药膏外涂以及得保松皮损内注射,治疗前及治疗后1、3、6个月各积分1次。6个月后评判临床疗效。实验研究选择活动期瘢痕疙瘩组织进行体外瘢痕疙瘩成纤维细胞培养,然后设五倍子瘢痕膏水溶液大中小3个剂量组,用MTT法观察对成纤维细胞增殖率的抑制作用,流式细胞仪检测药物对细胞DNA周期及细胞凋亡率的影响,用mRNA-DNA斑点杂交检测对瘢痕疙瘩成纤维细胞Ⅰ、Ⅲ前胶原基因表达的影响。
     结果:1.五倍子瘢痕膏治疗瘢痕疙瘩有效率为96.56%,疗效优于黑布药膏组(P<0.05),与得保松相比无差异,但副作用的发生率明显低于得保松组(P<0.01);2.五倍子瘢痕膏水溶液对瘢痕疙瘩的成纤维细胞的增殖率有一定的抑制作用,且有一定的剂量依赖关系;3.能促使瘢痕疙瘩成纤维细胞的凋亡;4.使瘢痕疙瘩成纤维细胞G_2M期和S期的细胞增加,出现S期阻滞,提示五倍子瘢痕膏能抑制瘢痕疙瘩成纤维细胞的增殖;5.对瘢痕疙瘩成纤维细胞Ⅰ、Ⅲ型前胶原基因表达有显著性的抑制作用。
     结论:五倍子瘢痕膏治疗瘢痕疙瘩有较好的临床疗效,且弥补了传统药膏剂型落后,易干燥、易脱落、存放时间短的缺陷,也没有得保松皮质激素的副作用,治疗瘢痕疙瘩有一定的优势,其作用机制与抑制瘢痕疙瘩成纤维细胞增殖及其Ⅰ、Ⅲ型前胶原基因表达和促进成纤维细胞凋亡有关。
Objective:To evaluate the clinic effect of Wubeizi Ointment(WBZ Ointment) on keloid and the effects of WBZ Ointment solution on fibroblasts proliferation,apoptosis,and typeⅠandⅢprocollagen gene expression.
     Method:39 patients with 82 keloids were selected,which were divided into 3 groups,treated respectively with WBZ Ointment,Heibu Ointment (HB Ointment) external use and Disprospan injection in in.jury skin,and the cumulative scoring were made before treatment and post treatment in 1~(st) month,3~(rd) month,and 6~(th) month respectively.6~(th) month late,the clinic effect were evaluated.In experiment research,keloid tissue of activity stage were selected for keloid-derived fibroblasts culture in vitro,and the WBZ Ointment solution were divided into three doses consisting of large, middle and small dose,which were applied to fibroblasts respectively. And the effects of the drugs on keloid fibroblasts proliferation was examined by MTT colorimetric cell proliferation assay,cell DNA period and apoptosis detected by flow cytometry,as well typeⅠandⅢprocollagen gene expression of keloid fibroblasts by mRNA-DNA dot blot hybridization.
     Results:1 The clinic effective rate of WBZ Ointment was 96.56%,which was equal to Disprospan treatment,both of them were better than HB Ointment(P<0.05).And compared to Disprospan,WBZ Ointment had little side effects(P<0.01) 2 WBZ Ointment solution restricted keloid fibroblasts proliferation with dose-dependent manner.3 WBZ Ointment solutions accelerated the keloid fibroblasts apoptosis.4 After WBZ Ointment solution application,the keloid fibroblasts which were in G2M stage and S stage increased,with S stage impaled,it suggested WBZ Ointment could restrict keloid fibroblasts proliferation.5 WBZ solution restricted typeⅠandⅢprocollagen gene expression of keloid fibroblasts significantly.
     Conclusion:WBZ Ointment has satisfied effect on keloid,and WBZ Ointment improved the lagged preparation and the defects of traditional ointment,such as easy to dry and fall off,short period to storage,and WBZ Ointment hasn't side effects like Disprospan.Therefore,it has some advantage on clinic application for keloid.The mechanism of WBZ Ointment for keloid involve in restricting keloid fibroblasts proliferation and typeⅠandⅢprocollagen gene expression,as well as accelerating keloid fibroblasts apoptosis.
