sICAM-1、sTM在抗磷脂综合征自然流产患者中的检测及意义

摘要
目的:通过检测抗磷脂综合征自然流产患者血清可溶性细胞间粘附分子1(sICAM-1)、可溶性血栓调节蛋白(sTM)的浓度,探讨二者在抗磷脂综合征发病机制中的作用。
     方法:
     将40例有自然流产史的APS患者作为研究组,33例有自然流产史的非APS患者作为对照Ⅰ组,20例正常无自然流产史的妇女作为对照Ⅱ组。采用酶联免疫吸附试验(ELISA)检测血清ACA、aβ2GPⅠ、sICAM-1、sTM浓度,自动血液流变仪检测血浆粘度。
     结果:
     1、研究组、对照Ⅰ组、Ⅱ组sICAM-1浓度分别为338.49±14.88ng/ml,259.50±14.17ng/ml,253.51±13.12ng/ml;sTM浓度分别为22.07±2.28ng/ml、14.15±0.90ng/ml、14.06±1.30 ng/ml,研究组sICAM-1、sTM浓度均显著高于对照Ⅰ组及Ⅱ组(P<0.001),对照Ⅰ组与Ⅱ组比较,差异均无统计学意义(P>0.05)。
     2、研究组ACA阳性患者ACA、sICAM-1、sTM浓度分别为16.27±2.93u/ml,341.45±16.65ng/ml、22.60±2.58 ng/ml,ACA与sICAM-1、sTM浓度的相关系数分别为0.467(P<0.05)、0.503(P<0.01),ACA与sICAM-1、sTM浓度均呈正相关。
     aβ2GPⅠ阳性患者aβ2GPⅠ、sICAM-1、sTM浓度分别为11.40±1.56 u/ml、343.69±14.67 ng/ml、22.67±2.15 ng/ml,aβ2GPⅠ与sICAM-1、sTM浓度的相关系数分别为0.835(P<0.001)、0.859(P<0.001),aβ2GPⅠ与sICAM-1、sTM浓度均呈正相关。
     3、研究组sICAM-1与sTM浓度相关系数为0.799(P<0.001),二者呈正相关。研究组血浆粘度为1.86±0.16 mPa·s,高于对照Ⅰ组及Ⅱ组(P<0.001)。sICAM-1与血浆粘度的相关系数为0.669(P<0.001),二者呈正相关。
     4、研究组治疗前ACA、aβ2GPⅠ、sICAM-1、sTM浓度、血浆粘度分别为13.08±5.04 u/ml、9.59±8.7 u/ml、338.49±14.88 ng/ml、22.07±2.28ng/ml、1.86±0.16 mPa·s;治疗后各指标分别为6.93±2.10 u/ml、5.04±1.74 u/ml、276.91±7.35 ng/ml、16.11.76ng/ml、1.63±0.05mPa·s。治疗后各指标浓度均显著低于治疗前(P<0.001)。
     结论:
     1、sICAM-1及sTM与APS有关,可引起血液高凝。
     2、sICAM-1及sTM可作为判断APS病情程度的指标。
Objects:To detect the concentration of soluble intercellular adhesion molecule-1(sICAM-1)and soluble thrombomodulin protein (sTM)in patients of antiphospholipid syndrome with spontaneous abortion,research their role of the pathogenesis of antiphospholipid syndrome.
     Methods:The serum samples,which were obtained from the study group,composed of 40 APS patients with spontaneous abortion,from the control groupⅠ,composed of 33 non- APS patients with spontaneous abortion,and from the control groupⅡ,composed of 20 normal women, were measured by Enzyme-Linked immunosorbant assay for ACA、aβ2GPⅠ、sICAM-1 and sTM.Plasma viscosity was measured by automatic-hemorheologic machine.
     Results:
     (1)SICAM-1 concentration of the study group,the control groupⅠandⅡwere 338.49±14.88 ng/ml,259.50±14.17ng/ml and 253.51±13.12ng/ml,sTM concentration in these three groups were 22.07±2.28ng/ml、14.15±0.90ng/ml and 14.06±1.30 ng/ml.The study group demonstrated significantly higher concentration of sICAM-1 and sTM than that of the control groupⅠandⅡ(P<0.001).Between the control groupⅠandⅡ,there have no significant difference(p>0.05)
     (2)In ACA positive patients of the Study Group,ACA、sICAM-1 and sTM concentration was 16.27±2.93u/ml、341.45±16.65ng/ml and 22.60±2.58 ng/ml.The Pearson correlation coefficient between ACA and sICAM-1、sTM was 0.467(P<0.05)、0.503 (P<0.01).The relationship of ACA and sICAM-1、sTM is positive correlation.
     In aβ2GPⅠpositive patients,aβ2GPⅠ、sICAM-1 and sTM concentration was 11.40±1.56 u/ml、343.69±14.67 ng/ml and 22.67±2.15 ng/ml.The Pearson correlation coefficient between aβ2GPⅠand sICAM-1、sTM was 0.835(P<0.001)、0.859(P<0.001)The relationship of aβ2GPⅠand sICAM-1、sTM is positive correlation.
     (3)In the study group,the Pearson correlation coefficient between sICAM-1 and sTM concentration was 0.799(P<0.001),the two are positive correlated.The plasma viscosity was 1.86±0.16 mPa-s,which demonstrated higher concentration than that of the control groupⅠandⅡ(P<0.001).The Pearson correlation coefficient between sICAM-1 and plasma viscosity was 0.669(P<0.001),the two are positive correlated.
     (4)In the study group,ACA、aβ2GPⅠ、sICAM-1、sTM concentration and plasma viscosity of before treatment was 13.08±5.04 u/ml、9.59±8.7 u/ml、338.49±14.88 ng/ml、22.07±2.28ng/ml、1.86±0.16 mPa·s;,and of after treatment was 6.93±2.10 u/ml、5.04±1.74 u/ml、276.91±7.35 ng/ml、16.11.76 ng/ml、1.63±0.05mPa·s.The group after treatment demonstrated significantly lower concentration of each than the the group before treatment(P<0.001).
     Conclusions:
     (1)SICAM-1 and sTM correlate with APS.They may cause blood hypercoagulability.
     (2)SICAM-1 and sTM can be used as a severity index of APS.
