纤维连接蛋白及重组FN肝素结合域多肽对血小板减少内毒素血症小鼠治疗作用的初步研究

摘要
纤维连接蛋白(Fibronectin,FN)是细胞外基质的重要成份,为一种具有多种功能的糖蛋白大分子,能与肝素、胶原、纤维蛋白以及整合素受体结合,参与细胞的粘附、迁移、止血、损伤修复等过程、在抗感染、维持微血管的完整性等方面具有重要作用。大量研究表明血浆FN水平的测定对感染的预后评价具有重要意义。严重感染,如败血症患者血浆FN水平明显且急剧降低,给其补充富含FN的冷沉淀素,则在患者血浆FN恢复的同时,伴有心肺功能的改善及内毒素血症的控制,死亡率也随之下降。我们前期工作表明FN能降低半乳糖胺敏化内毒素小鼠的死亡率,有助于改善内毒素血症的病情及预后。重组FN肝素结合域多肽(rhFNHN-29)位于FN的N端,由五个I型的同源结构组成,该结构域的分子量为29kDa,由237个氨基酸组成,可与肝素结合。应用FN肝素结合域多肽治疗鼠内毒素血症和血小板减少模型,目前国内外尚未见报道。在本研究中,为了更真实地反映血液病患者常见的感染和出血并存的情况,我们设计了血小板减少内毒素血症小鼠模型,观察FN及rhFNHN-29对其疗效作用,并初步探讨其作用机制,为FN及rhFNHN-29的临床应用提供理论及实验依据。
     本文的研究内容主要包括:
     1.血小板减少内毒素血症小鼠模型的构建
     每只小鼠腹腔注射血小板抗血清(APS)100μl,30分钟后腹腔注射500μl含内毒素脂多糖(LPS)300μg加半乳糖胺(GalN)10mg的混合液。通过血小板监测和72小时后小鼠的肝脏组织病理形态学观察验证模型是否成功构建。
     2.FN及rhFNHN-29对血小板减少内毒素血症小鼠的作用,并初步探讨其作用机制
Fibronectin is known to be a large multifunctional glycoprotein with binding sites for many substances, including heparin, collagen, fibrinogen, fibrin and integrin. It is involved in a variety of cellular processes, such as cell migration, adhesion, hemostasis and tissue repair. And it plays an important role in maintaining the integrity of blood capillary and anti-infection. Many clinical studies suggested that plasma fibronectin concentration might be a prognostic index for endotoxemia. Clinical trails also suggested that reversal of plasma FN deficiency by infusion of FN-rich cryoprecipitate might result in a marked improvement of cardiopulmonary function in the endotoxemia patients. Our previous studies indicated that plasma FN could prevent hepatic failure induced by endotoxin in mice. Recombinant heparin-binding domain of FN is located at its amino-terminal, consisting of 5 typeⅠrepeats with 237 amino acids. There had been few reports in the world that the heparin-binding domain in intact FN molecular structure could be used in the treatment of endotoxemia with thrombocytopenia in mice. In the studies, we designed the model of endotoxemia with thrombocytopenia for actual reproduction of the concomitance state of endotoxemia and thrombocytopenia in hematologic disease, and investigate the therapeutical effect of FN and rhFNHN-29 on this state.
