基于穴位脏腑相关性的足三里穴对胃肠功能紊乱大鼠的调节作用及机制研究

摘要
目的:通过观察电针足三里穴对正常及胃肠功能紊乱大鼠的调节作用探讨足三里穴与胃特异性的效应机制。
     方法:80只SD大鼠随机分为正常和模型两部分,腹腔注射利血平制作胃肠功能紊乱模型,两种方法电针足三里、三阴交穴,共10组,每组8只。激光多普勒微循环血流分析仪监测胃、肠、肝、脑等器官表面微循环。多道生理记录仪监测胃肠生物电。高效液相、放免法检测血液及组织中5-HT、DA、NE、E、MTL、GAS、ET等神经递质、胃肠激素含量。免疫组化法检测5-HT、VIP在胃、肠、脑组织中的表达。原位杂交法检测VIPR_2 mRNA在胃、肠、脑组织中的表达。
     结果:对于正常大鼠,电针能升高胃、肠微循环,足三里2组尤为显著(P<0.05);能使胃肠生物电振幅降低,频率增快,足三里组胃电频率较三阴交组明显增快,有显著性差异(P<0.05);血浆ET含量降低(P<0.05),升高血及组织中MTL、GAS含量(P<0.05),血清及下丘脑中NE、E、DA含量升高,但对5-HT影响不明显;胃、肠、脑组织中的5-HT、VIP、VIPR_2 mRNA表达升高(P<0.05)。
     对于模型大鼠,电针能升高降低的胃、肠微循环,较三阴交穴,足三里穴升高胃微循环更明显(P<0.05);能使胃肠生物电振幅降低,频率增快;血浆ET含量降低(P<0.05),升高血及组织中MTL含量,升高血、脑GAS含量,降低胃GAS含量,可升高E、DA含量,升高下丘脑5-HT含量;降低血清5-HT含量及组织中的表达(P<0.05),可升高模型大鼠降低的VIP及其受体的表达(P<0.05)。
     结论:电针足三里穴能有效调节正常及胃肠功能紊乱模型大鼠胃肠微循环、生物电及神经递质、胃肠激素等相关指标的变化,具有一定的穴位特异性,提示足三里穴与胃之间存在特异性关系,为穴位脏腑相关性理论提供一定科学依据。
Objective:To explore the special interation between Zusanli and stomach through observing the regulating effect and mechanism of electroacupuncture(EA) at Zusanli in normal and model rats with functional gastrointestinal disorders(FGID).
     Methods:80 SD rats were randomly divided into normal and model,included 10 groups with 6 cases in each according to the difference of Zusanli and Sanyinjiao.FGID model was established by intraperitoneal injection of reserpine.EA was carried on Zusanli and Sanyinjiao.The microcirculatory blood flow was observed with a laser doppler blood flow monitor in the surface of stomach,duodenum,liver and brain,and the bioelectrical feature of stomach and intestine detected with multi track polygraph.The neurotransmitter and gut hormone in the blood and tissues such as 5-HT,DA,NE,E,MTL,GAS and ET were detected with the methods of radioimmunity and high performance liquid chromatography.Methods of immunohistochemistry and in situ hybridization were used to detect the expression of 5-HT,VIP and VIPR_2 mRNA.
     Results:For the normal rats,the microcirculatory of stomach and intestine was improved by EA,especially Zusanli 2 group(P<0.05).The amplitude was decreased,while the frequency of bioelectricity was increased,Zusanli is more significant than Sanyinjiao at the frequency(P<0.05).The content of ET was reduced by EA(P<0.05),the content of MTL,GAS was enhanced,the content of NE,E and DA was raised by EA,while 5-HT was not significant.The expression of 5-HT,VIP,VIPR_2 mRNA in normal stomach was enhanced in stomach,intestine and brain(P<0.05).For the model rats,the descendent microcirculatory of stomach and intestine was improved by EA,and Zusanli was more apparente than Sanyinjiao in the microcirculatory of stomach.The amplitude was decreased,while the frequency of bioelectricity was increased,The ascendant content of ET was reduced(P<0.05),the content of MTL was enhanced,the serum and intestine content of GAS was increased,the stomach's was reduced,the content of E,DA,5-HT in hypothalamus was raised,the serum content of 5-HT was reduced by EA.The ascendent expression of 5-HT was depressed,and the decendant expression of VIP,VIPR_2 mRNA was enhanced in stomach,intestine and brain of the model rats by EA(P<0.05).
     Conclusion:Microcirculation,bioelectricity and related biochemical indicators of the normal and model rats with FGID can be effectively regulated by EA at Zusanli.That may be verified that Zusanli is a certain specific point to stomach and has special relations between them,which provided scientific evidence for the theory of acupoint and visceral coupling.
