Vypro Ⅱ补片在腹股沟疝修补术后疗效的系统评价

摘要
目的比较VyproⅡ片与普通聚丙烯补片在腹股沟疝修补术后的临床疗效。
     简要背景在腹股沟疝手术中,假体补片的应用变得越来越普遍,以前的许多RCT已经比较了VYPROⅡ补片与普通聚丙烯补片的相对优缺点,现在需要一个系统评价来综合过去的研究成果。
     方法检索PubMed (1966-2009.05), Cochrane library(2009年第2期),EMBASE (1974-2009.05), SCI(1974-2009.05),中国生物医学文献数据库(CBM)(1978-2009.05)等数据库收录的公开发表的关于VyproⅡ补片的临床随机和半随机对照试验。由两名研究者独立对符合标准的研究进行资料提取。采用Cochrane系统评价手册简单评分法评价纳入文献质量和Cochrane协作网提供的RevMan 5.0.1统计软件进行Meta分析。计量资料采用加权均数差(WMD)或标准化均数差(SMD)计算统计量,计数资料用相对危险度(RR)或优势比(OR)计算统计量,各效应量均以95%可信区间(CI)表示。评价指标包括:术后疝复发、术后慢性疼痛、异物感、尿道感染、血清肿和睾丸萎缩。
     结果10个RCTs,2027例患者纳入分析。Meta分析显示:与普通聚丙烯补片比较,VyproⅡ补片在术后疝复发[OR=1.53,95%CI(0.73,3.19)],术后慢性疼痛[OR=0.87,95%CI(0.55,1.37)],尿道感染[OR=0.71,95%CI(0.14,3.63)],血清肿[OR=0.80,95%CI(0.26,2.48)]和睾丸萎缩[OR=1.94,95%CI(0.58,6.49)]方面均无统计学差异,但异物感发生率明显较低[OR=0.58,95%CI(0.42,0.80)]。
     结论目前证据显示,除VyproⅡ补片在异物感方面占优外,VyproⅡ补片与普通聚丙烯补片在腹股沟疝修补术后的短期临床疗效无明显差别,期待更多高质量、随访期长的相关随机对照试验进一步验证。
Objective:The aim of this Meta Analysis was to compare the effectiveness between Vypro II mesh and polypropylene mesh after inguinal hernia repair.
     Summary Background Data:The use of prosthetic meshes had become increasingly popular performing inguinal hernia surgery. Previous RCTS had shown their relative merits and risks between the VYPROⅡmesh and the standard polypropylene mesh. A Meta Analysis was needed to estimate the real effectiveness.
     Methods:PubMed (1966-2009.05), Cochrane library (2 issue,2009), EMBASE (1974-2009.05), SCI (1974-2009.05) and Chinese Biomedical Literature Database (1978-2009.05) were used to search the published clinical randomized controlled trials about VyproⅡmesh. Two independent reviewers assessed trials for eligibility and quality, all the related data matched our standards were abstracted for Meta-analysis by RevMan 5.0.1. Weighted mean difference (WMD) or standard mean difference (SMD) between continuous variables was calculated. For dichotomous data, relative risk (RR) or odds radio (OR) was determined. All effect values were expressed by 95% confidence intervals (CI). The evaluation criteria included recurrence, pain, feeling of foreign body, urinary tract infection, seroma and testicular atrophy.
     Result:2027 patients of 10 RCTs were included. Compared with polypropylene Mesh, VyproⅡMesh had no significant difference in recurrence [OR=1.53,95%CI (0.73,3.19)], pain [OR=0.87,95%CI (0.55,1.37)], urinary tract infection [OR=0.71, 95%CI (0.14,3.63)], seroma [OR=0.80,95%CI (0.26,2.48)] and testicular atrophy [OR=1.94,95%CI (0.58,6.49)], but feeling of foreign body was significantly lower [OR=0.58,95%CI (0.42,0.80)].
     Conclusion:Current evidence suggests that there is no significant difference between VyproⅡand polypropylene mesh in the short term effectiveness. However, use of VyproⅡmesh was associated with less feeling of foreign body. Further high-quality, long follow-up period RCTs should be carried out to provide more reliable evidence.
