针刺治疗偏头痛的临床疗效及生化学机制研究

摘要
偏头痛(Migraine)是一组常见的头痛类型,为发作性神经
    ——血管功能障碍,以反复发生的偏侧或双侧头痛为特征,其病
    因和发病机制十分复杂,迄今尚不完全清楚。该病属于祖国医学
    “偏头风”、“偏头痛”范畴。
     现已证实,偏头痛与血小板聚集及血管舒缩功能障碍有关,
    而血小板活化因子(PAF)、血栓烷A_2(TXA_2)、前列环素(PGI_2)
    均与这两者关系密切。本文对63例偏头痛患者进行了观察,将
    病人随机分为针刺组(32例)和对照组(31例),观察临床疗效
    及治疗前后血中PAF、TXB_2、6—酮—PGF_(1α)含量,并选择20名
    健康人作为正常对照组。结果显示:针刺治疗对偏头痛疗效明显,
    总有效率达90.6%,优于对照组(P<0.01);偏头痛患者在治疗前
    血中PAF、TXB_2、6—酮—PGF_(1α)含量均高于正常对照组(P<0.01),
    而针刺组在治疗后血中PAF、TXB_2、6—酮—PGF_(1α)含量低于对照
    组(P<0.05,P<0.01)。实验结果表明:针刺治疗偏头痛的疗效明
    显,优于对照组;PAF、TXB_2、6—酮—PGF_(1α)均参与偏头痛的病理
    过程,针刺对偏头痛患者血中PAF、TXB_2、6—酮—PGF_(1α)有调节作用。
    本文还对针刺治疗偏头痛的机理进行了探讨,为临床治疗提供了
    理论依据。

     Migraine is a common type of the headache,characterised as the
     repeated episodes of one or tow sides head pain,caused by the
     dysfunction of nerve and vessel.This disease is classified as擯ian
     Tou Fen攁nd Migraine in the category of the traditional chinese
     medicine.The etiology and mechanism of the disease are very
     complicated and unknown.At present,the Migraine is shown as the
     relation to the function of the platelet collection,the systole and
     dilatation of vessel,as well as PAF,TXA2,PGI2.In this paper,63
     patients with Migraine have been taken and divided into 2 groups,
     acupuncture group (32cases) and the control (3 leases), the effect of
     the clinic therapy observed after treatment, the level of PAF,TXB2,
     6?keto?PGF1 in the plasma of patients observed before and after
     treatment,the normal control,20 cases.The result show that Migraine
     treated with acupuncture therapy get obvious effect which is better
     than that of the control抯(P     the level of PAF,TXB2,6梜eto桺GF10 after treatment is lower than
     that of before treatment (P<0.0l),and that of acupuncture group is
     the lowest (P<0.Ol,O.05).All above suggest that PAF,TXB2,6梜eto
     桺GF1 0ve relation to pathogenesis of Migraine and be adjusted
     by acupuncture.Migraine treated with acupuncture therapy get
     obvious effect which is better than that of the control抯.The
     mechanism of Migraine with acupuncture treatment has been
     discussed for the clinic therapy in this paper, too.
