艾灸对慢性疲劳模型大鼠神经内分泌免疫系统的影响

摘要
研究目的:
     慢性疲劳综合征(Chronic Fatigue Syndrome,简称CFS)是由美国疾病控制中心于1988年正式命名。目前,CFS发病率较高,并有逐年升高的趋势,严重影响人们的生活质量。祖国医学在本病的治疗上有一定优势,本实验通过建立复合应激大鼠模型模拟CFS的临床表现,并对其进行艾灸治疗,通过对比观察各组大鼠的行为、生化指标,肾上腺皮质激素(ACTH)、皮质酮(CORT)、促肾上腺皮质激素释放激素(CRH)、细胞因子IL-1β、IL-6的含量来探讨CFS的发病机理、艾灸的治疗效果和作用机理,以期为临床治疗慢性疲劳综合征提供一种有效的治疗方法。
     研究方法:
     将50只大鼠随机分为正常组、模型组和艾灸组。适时饲养1周,第8天开始对模型组和艾灸组大鼠采用复合应激方法(剥夺睡眠20h和力竭游泳法)建立慢性疲劳模型,共3周,正常组不予任何刺激,正常喂养。造模结束后第二天,艾灸组取大鼠双侧肾俞、足三里穴,点燃施灸,每天每穴灸8min,每周治疗5天,连续治疗3周。正常组和模型组大鼠不实施任何处理方法。造模前后、治疗结束后,分别对各组大鼠进行行为学观察;治疗结束后检测对比各组大鼠的生化指标和ACTH、CORT、CRH、IL-1β、IL-6的含量。
     研究结果:
     1、造模结束后,模型组和艾灸组大鼠均比正常组大鼠的体重轻、力竭游泳时间短、鼠尾悬挂不动时间长、水平运动和垂直运动得分少,差异有统计学意义(P<0.05),说明大鼠出现了躯体疲劳和心理疲劳。治疗结束后,艾灸组比模型组的体重重、力竭游泳时间长、鼠尾悬挂不动时间短、水平运动和垂直运动得分多,差异有统计学意义(P<0.05),说明艾灸足三里、肾俞穴可以改善模型大鼠的体力,调整抑郁情绪,缓解大鼠的心理疲劳和躯体疲劳。
     2、治疗结束后,模型组大鼠的血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、白蛋白(ALB)、尿素氮(BUN)、尿酸(UA)、L-乳酸脱氢酶(LDH)、肌酸激酶(CK)与正常组大鼠比较,差异有统计学意义(P<0.05)。而血清总蛋白(TP)、球蛋白(GLB)、肌酐(CREAT)、葡萄糖(GLU)、总胆固醇(TCHO)、甘油三酯(TG)、C反应蛋白(CRP)含量与正常组比较,差异无统计学意义(P>0.05)。说明复合应激造模因素影响CFS大鼠的部分生化指标。
     3、艾灸组大鼠的AST、ALB、BUN、LDH、CK与模型组大鼠比较,差异有统计学意义(P<0.05)。而艾灸组大鼠的ALT、UA与模型组比较,差异无统计学意义(P>0.05)。说明艾灸足三里、肾俞穴能够使部分生化指标有所恢复。
     4、模型组大鼠血清ACTH、CRH、CORT的含量情况与正常组比较,差异有统计学意义(P<0.05),说明模型大鼠的下丘脑-垂体-肾上腺素轴功能亢进。
     5、艾灸组大鼠的血清ACTH、CRH、CORT与模型组比较,差异有统计学意义(P<0.05)。说明艾灸能够调节下丘脑-垂体-肾上腺轴的亢进状态。
     6、艾灸组大鼠细胞因子IL-1β、IL-6与模型组比较,差异有统计学意义(P<0.01)。说明艾灸足三里、肾俞穴能够调节IL-1β、IL-6的含量。
     研究结论:
     1、本实验采用复合应激造模因素(剥夺睡眠20h和强制游泳)复制CFS大鼠模型,造模后模型大鼠消瘦明显,力竭游泳时间缩短,鼠尾悬挂时间延长,水平运动和垂直运动得分较少,与CFS患者的临床表现类似,动物模型复制成功。
     2.艾灸足三里、肾俞穴能够明显改善慢性疲劳大鼠疲劳状态,恢复部分生化指标,是治疗CFS的有效方法。
     3、HPA轴功能亢进和免疫系统的变化,可能都是CFS的发病机制。
     4、免疫细胞与下丘脑-垂体-肾上腺轴之间存在着密切的联系,神经系统、内分泌系统、免疫系统之间相互影响。
     5、艾灸足三里、肾俞穴能够改善HPA轴的亢进状态,调节免疫系统,提示艾灸治疗CFS的作用机制是多途径的整体调节。
Objective
     Chronic Fatigue Syndrome was named by DCD in1988. Now, the incidence of CFS is high, and higher and higher year by year. CFS seriously affects people's quality of life. Chinese Traditional Medicine in treatment of it has a certain advantage. We set up CFS rat model by chronic multiply factors to simulate the clinical manifestations of CFS and treated models by moxibustion. the experiment researched the effect and mechanism of therapy of moxibustion by observing the behavior, biochemical indexes and the content of the serum ACTH, CORT, CRH,IL-1β, IL-6of each group rats, to achieve an effective treatment method of chronic fatigue syndrome
     Method
     We divided50Wistar rats into the moxibustion group as the threrapy group, the normal group and the model group randomly. All rats were feeded a week, To set up CFS rat model, the rats of the model group and the moxibustion group were deprived sleep for20hours and forced to swim exhaustedly in eighth days, for3weeks. The normal group' rats were feeded normal without any stimulation. The next day of end of setting up model, the moxibustion group'rats were treated by moxibustion on the acupoints of Zusanli and Shenshu, Each acupoint was treated for eight minutes every day, five days a week, treated for three weeks continuously. The rats of the normal group and the model group weren't treated.
     We obeserved the behavior of each group rats before the setting up model and after and the end of treatment, and detected the biochemical indexes and the content of the serum ACTH, CORT,CRH.
     Result
     1. The end of setting up model, the weigh, exhausting swimming time, rat tail suspension time, horizontal movement and vertical motion of the model group and moxibustion group'rats was lighter, shorter, longer, less than the normal group, the difference was statistically significant (P<0.05), which shows that the model rats appear the physical fatigue and mental fatigue. After the treatment, the weigh、exhausting swimming time、rat tail suspension time, horizontal movement and vertical motion of the moxibustion group'rats was heavier,longer, shorter, more than the model group, the difference was statistically significant (P<0.05), which shows that moxibustion on acupoints of Zusanli and Shenshu can improve model rats' physical strength, adjust their depression.
     2. After the treatment, the content of the serum ALT, AST, ALB, BUN, UA, LDH, CK of model group' rats were compared to the normal group, the difference was statistically significant(P<0.05). The content of the serum TP, GLB, CREAT, GLU、 THCO, TG, CRP of model group' rats were compared to the normal group, the difference was'nt statistically significant (P>0.05), which shows that multiple factors may affect the biochemical indexes of CFS rats.
     3. The content of the serum ALT, AST, ALB, BUN, UA, LDH, CK of model group'rats were compared to the normal group, the difference was statistically significant (P<0.05). While the ALT and UA of both group's haven't significant difference.
     4. The content of the serum ACTH,CRH, CORT of the model group'rats was compared with the normal group, the difference was statistically significant (P<0.05), which shows that hypothalamus-pituitary-adrenergic axis'function of model rats is in condition of hyper.
     5. The content of the serum ACTH, CRH, CORT of the moxibustion group'rats was compared with the model group, the difference was statistically significant (P<0.05), which shows that moxibustion on the acupionts of Zusanli and Shenshu can regulate the function of the hypothalamus-pituitary adrenergic axis.
     6. The content of the serum IL-1β, IL-6of the moxibustion group'rats was compared with the model group, the difference was statistically significant (P<0.01), which shows that moxibustion on the acupionts of Zusanli and Shenshu can regulate the function of the immunecell IL-1β, IL-6.
     Conclusion
     1. We set up CFS rat model by multiple factors, After treatment, model rats'weigh was ligther, exhausting swimming time was shorter, rat tail suspension time was longer, horizontal movement and Vertical motion were less, which was similar to the clinical presentation of CFS, so, the setting up model was success.
     2. Moxibustion on the acupoints of Zusanli and Shenshu can improve the fatigue state of chronic fatigue rats significantly, recover certain biochemical indexes, so, which is an effective treatment of CFS.
     3. The hyperactivity of HPA axis and the change of immune system may be the pathogenesis of CFS.
     4. There is a close contact between immunecells and the hypothalamus-pituitary-adrenalaxis, and the nervors system, endocrine System, immune system interact.
     5. Moxibustion on zusanli and shenshu can improve HPA axis hyperactivity state, modulate the immune system, which suggests that the mechanism of moxibustion treatment for CFS is in many ways of regulating the whole.
