全反式维甲酸对兔后发性白内障的抑制作用

摘要
目的探讨全反式维甲酸(all-trans retinoic acid,ATRA)抑制兔后发性白内障的作用。
     方法将12只健康、眼部各项检查均正常的家兔(24只眼)按照随机分组的方法分为空白对照组和药物实验组,每组6只家兔(12只眼),在全麻下对两组家兔眼行透明晶状体囊外摘除术,药物试验组灌注液中加入全反式维甲酸,空白对照组灌注液为常规BSS液。分别于术后1、3、7、14、30d和60d对所有术眼进行裂隙灯检查和眼底检查,并抽取家兔眼房水于-80℃中保存。用酶联免疫吸附试验(ELISA)对透明晶状体囊外摘除术后两组家兔眼房水中的白细胞介素-1β(IL-1β)和转化生长因子-β(TGF-β)含量进行检测。3月后处死动物,对2组家兔眼分别行组织病理学观察和电镜观察。
     结果透明晶状体囊外摘除术后检测两组家兔眼房水中IL-1β和TGF-β的含量,药物实验组明显低于空白对照组,差异有显著学意义(P<0.001)。透明晶状体囊外摘除术后3月药物实验组晶体后囊膜透明度优于空白对照组(P<0.05)。空白对照组晶状体后囊膜前覆盖了一层晶状体上皮细胞,药物实验组晶状体后囊膜前无明显晶状体上皮细胞的增殖。
     结论ATRA可抑制房水中IL-1β和TGF-β的表达,能够有效抑制兔眼后发性白内障的发生。
Objective To explore the inhibitive effect of all-trans retinoic acid (ATRA) on preventing posterior capsule opacification in rabbit eyes.
     Methods Extracapsular cataract extraction was undertaken on 12 rabbit eyes which were divided into two groups:control group and ATRA group.The ATRA group were using ATRA in the operation, the control group were using BSS.The aqueous humors were collected at 1 day,3,7, 14,30 and 60 days after operation and investigated the concent ration of IL-1βand TGF-βby enzyme linked immunosorbent assay (ELISA). Three months later, the biomicroscopic photograph of posterior capsule were observed by lighe microscopy examination and the pathological changes of posterior lens capsule were observed by electron microscopy examination.
     Results The content of IL-1βand the content of TGF-βin ATRA group were significantly lower than those in control group, showing a statistically significant difference (P< 0.001). In the 3 month after the operation,the eye numbers which developed posterior capsule opacification (PCO) were 12 and 4 in control group and ATRA group, respectively with a significant difference between control group and ATRA group (P< 0.05). No or only sporadic lens epithelial cells and fibrin sticking were found on posterior capsule of lens in ATRA group under the light and electron microscope, and severe PCO including lots of poradic lens epithelial cells and fibrin sticking was seen in control group.
     Conclusion The implantation of ATRA into the anterior chamber during the ECCE operation may prevent the occurrence of PCO to some extent.
