温肾化瘀法治疗高尿酸血症及痛风性关节炎的机理探讨

摘要
目的:研究温肾化瘀法治疗高尿酸血症及痛风性关节炎的作用机理。
     方法:将温肾化瘀中药分别用于高尿酸血症动物模型和痛风性关节炎动物模型身上,并与降尿酸药(苯溴马隆和别嘌呤醇)和抗炎药(消炎痛)对照观察其对模型动物血尿酸水平的影响,并评价其抗炎镇痛效果。
     结果:在高尿酸血症动物模型中,温肾化瘀中药能明显降低异常升高的血尿酸和尿素氮(P<0.05),但对黄嘌呤氧化酶活性的影响与阳性对照组比较未作出统计学意义(P>0.05)。在痛风性关节炎动物模型中,我们分别通过鼠耳肿胀法和足跖肿胀法、测定细胞因子水平以及扭体法、热板法、疼痛介质水平的测定,证明了温肾化瘀中药具有明显的抗炎镇痛效果(P<0.05)。
     结论:温肾化瘀中药很可能有抑止黄嘌呤氧化酶(XOD)的活性和促进尿酸排泄的作用,其抗炎效果可能主要与抑止细胞因子释放有关,而镇痛作用可能主要与抑止外周K~+、DA和5-HT释放有关。病理切片和电镜检查肯定了温肾化瘀中药治疗痛风性关节炎的确切作用。
Objective: The aim of study is to research the functional mechanism oftherapy for hyperuricuria and gouty arthritis through warming kidney andremoving blood stasis.Method: The herb is used in the model of animal which get hyperuricuria orgouty arthritis. We evaluate the herb's curative effect through comparing withBenzbromarone,Allopurinol and Indometacin.Results: We can know the herb with the function of decreasing animal'sblood uric acid and urinary nitrogen level(p<0.05). We use the method of thetumefaction on the toe or ear of rats test, cytokine level, the writhing test, thehot plate test and the pain transmitter level,and prove herb with the effect ofanti-inflammation and analgesia too.Conclusion: The herb's anti- hyperuricuria function possibly inhibit xanthineoxidase activity and uric acid ejection. The anti-inflammation and analgesiamechanism may be related to inhibiting the cytokine and the pain transmitterreleasing. Pathological examination and electron microscope support herb'stherapy action.
