2型糖尿病患者微量白蛋白尿(MAU)水平与颈动脉内中膜厚度(CIMT)相关性研究

摘要
实验目的:研究2型糖尿病患者微量白蛋白尿(MAU)水平与颈动脉内中膜厚度(CIMT)的关系。
     方法:选取2型糖尿病129例,根据24小时尿白蛋白排泄率分为尿蛋白正常组75例(24h尿微量白蛋白<30mg/24h),微量尿蛋白组54例(24h尿微量白蛋白30mg~300mg/24h)。并根据微量白蛋白水平将后者分为3个亚组,应用颈部血管彩色超声检测患者双侧颈动脉内中膜厚度(CIMT)及斑块情况,观察尿蛋白正常组与微量尿蛋白组及微量尿蛋白组内3个亚组间颈动脉内中膜厚度的变化,以及年龄、体重指数、血压、高血压病史、血糖、糖化血红蛋白、血脂水平、糖尿病病史、斑块有无等方面的差异。
     实验结果:两组CIMT比较有显著差异(P <0.05),各亚组间CIMT比较有显著性差异(P <0.05);各组间有斑块人数占该组人数百分比的差异有统计学意义(P <0.05);两组空腹血糖、餐后2小时血糖、糖化血红蛋白、TC、糖尿病病史及血压、高血压病史比较差异有统计学意义(P <0.05);两组年龄、BMI、TG、HDL、LDL比较差异无统计学意义(P>0.05);从Spearman相关系数来看,2型糖尿病患者的MAU与CIMT呈显著正相关(r=-0.755、P<0.01)。
     结论:2型糖尿病患者颈动脉内中膜厚度与尿微量白蛋白水平有明显相关性。
Objective: To investigate the relationship between microalbuminuriaand carotid intima media thickness in type2diabetes mellitus.
     Methods: One hundred and twenty nine patients with type2diabetesmellitus,were divided into two groups according to the24hours’microalbuminuria.The seventy-five patients in normal albumininuria group had24hours’ microalbuminuria <30mg/dl and the fifty-four patients inmicroalbuminuria group had24hours’ microalbuminuria30mg/dl~300mg/dl.And the patients in microalbuminuria group were divided into three groupsaccording to the level of microalbuminuria. Underwent neck vascular colorDoppler ultrasonography to evaluate carotid intiama media thickness and thepresence of coronary atherosclerotic plaque,The carotid intima mediathickness(CIMT), age, body mass Index(BMI),blood pressure, the history ofhigh blood pressure, blood glucose, glycosulated hemoglobin, blood lipid,history and the presence of coronary atherosclerotic plaque were comparedbetween groups.
     Results: There was significant difference between groups about theCIMT(P<0.05). There was significant difference between groups of thepercentage of the patients who had coronary atherosclerotic plaque (P<0.05).There was also significant difference between the two groups of FBG,PBG,glycosulated hemoglobin, TC, history, blood pressure, and the history of highblood pressure (P<0.05). There was no significant difference between groupsof age,BMI, TG, HDL, LDL (P>0.05). Spearman correlation coefficientrevealed that there was significant correlation between microalbuminuria andcarotid intima media thickness in type2diabetes mellitus.
     Conclusion: There was significant correlation between microalbum -inuria and carotid intima media thickness in type2diabetes mellitus.
