GSTT1基因多态性和吸烟与肺癌遗传易感性关系的研究

摘要
目的:探讨谷胱甘肽S-转移酶T1基因多态性与肺癌遗传易感性的关系及谷胱甘肽S-转移酶T1基因多态性和吸烟在肺癌易感性中的交互作用。
     方法:采用病例对照研究和聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法对100例肺癌患者和135例健康对照者外周血的GSTT1基因型进行检测。
     结果:
     1.GSTT1(-)基因型在肺癌组和对照组分别为57%(57/100)和41.5%(56/135),二者间有显著性差异(OR=1.919,95%CI=1.128~3.267,P=0.016)。
     2.GSTT1(-)基因型患肺鳞癌的风险显著增加(OR=2.699,95%CI=1.130~6.449,P=0.025)。
     3.GSTT1(-)基因型的分布不存在性别上的差异(肺癌组OR=1.285,95%CI=0.578~2.859,P=0.538;对照组OR=1.038,95%CI=0.522~2.066,P=0.915)。
     4.携带GSTT1(-)基因型的吸烟者较不吸烟者患肺癌的风险增加3.643倍(95%CI=1.269~10.461,P=0.016)。
     5.吸烟者携带GSTT1(-)基因型较携带GSTT1(+)基因型患肺癌的风险增加3.286倍(95%CI=1.410~7.655,P=0.006)。
     6.吸烟量≥20包年的人群中携带GSTT1(-)基因型者较携带GSTT1(+)基因型者患肺癌的风险增加7.81倍(95%CI=2.122~28.749,P=0.002)。
     7.携带GSTT1(-)基因型并吸烟≥20包年者较携带GSTT1(-)基因型并吸烟<20包年者患肺癌的风险增加5.517倍(95%CI=1.060~28.716,P=0.042)。
     结论:
     1.GSTT1(-)基因型在肺癌患者中发生频率增高,能够增加肺癌遗传易感性,但GSTT1(-)基因型的分布不存在性别上的差异。
     2.GSTT1(-)基因型可能是肺鳞癌的易感性基因型。
     3.吸烟与GSTT1(-)基因型间可能存在交互作用,二者协同增加患肺癌的风险性;吸烟量越大且携带GSTT1(-)基因型者患肺癌的风险越高。
Objective:To investigate the association between the genetic polymorphism of GSTT1 and the susceptibility to lung cancer and the interactions between genetic polymorphisms and smoking on lung cancer susceptibility.
     Methods:A case-control study and PCR—RFLP techniques were used to identify GSTT1 genotype in 100 cases of lung cancer and 135 controls.
     Results:1.The frequency of GSTT1(-) genotype in lung cancer group and controls was 57%(57/100) and 41.5%(56/135) respectively(OR=1.919,95%CI= 1.128~3.267,P=0.016).2.The individuals carried GSTT1(-) genotype remarkably increased the risk of lung squamous cell carcinoma(OR=2.699, 95%CI=1.130~6.449,P=0.025).3.There were no gender differences of GSTT1 (-) genotype distribution.4.In people with GSTT1(-) genotype,smoking markedly increased the risk for lung cancer(OR=3.643,95%CI=1.269 10.461,P=0.016).5.In smokers,GSTT1(-) genotype significantly increased the risk for lung cancer(OR=3.286,95%CI=1.410~7.655,P=0.006).6.In smokers≥20 packyears,GSTT1(-) genotype remarkably increase the risk of lung cancer(OR=7.81,95%CI=2.122~28.749,P=0.002).7.In people with GSTT1(-) genotype,smokers≥20 packyears remarkably increase the risk of lung cancer(OR=5.517,95%CI=1.060~28.716,P=0.042).
     Conclusions:1.The distribution frequency of GSTT1(-) genotype was higher in lung cancer.May increase the genetic susceptibility to lung cancer, There were no gender differences of GSTT1(-) genotype distribution.2. GSTT1(-) genotype was associated with the risk for lung squamous cell carcinoma.3.There may be a synergetic interaction between smoking and GSTT1(-) genotype.4.The more the cigarettes smoke,the higher risk of lung cancer increased in those people who are smokers with GSTT1(-) genotype.
