即刻种植即刻负重中PRF对软硬组织的修复促进作用

摘要
目的本研究通过建立即刻种植即刻负重种植体修复的动物实验模型,观察即刻种植即刻负重中应用富血小板纤维蛋白(PRF)种植体周围软硬组织的生长情况。
     方法拔除5只beagle犬双侧下颌第四前磨牙(PM4),每只实验动物下颌左右两侧立即对称植入1枚柱状螺纹纯钛种植体并即刻负重。采用自身对照方式,实验侧采用PRF填塞种植体颊侧缺损,远中间隔缺损和拔牙窝;对照侧种植体颊侧缺损不填任何填充物,间隔不制备缺损,拔牙窝用明胶海绵填塞。术后第二天开始观察软组织的愈合情况;种植术后当日和3个月时拍摄X线根尖片,观察X线根尖片上种植体周围骨的新生;3个月后手术切开观察骨缺损位置新骨形成情况,并测量记录,同时切取双侧种植体颊侧缺损、间隔和拔牙窝的骨质,制作HE切片并在光镜下观察种植体骨组织切片。
     结果种植后实验组软组织的愈合时间平均为5.80±0.84天,对照组为8.20±0.45天。种植三个月后X线片可见实验组比对照组影像致密。实验组颊侧骨量增加高度平均为2.60±1.19mm,对照侧为1.60±1.52mmm,三个月后实验组种植体颊侧缺损面积平均为1.45±1.46 mm2,对照侧为3.00±2.15 mm2。光镜下观察骨切片,发现实验组种植体颊侧比对照组成骨细胞多,间隔骨成熟度与对照组无明显差别,远中拔牙窝处比对照组成骨细胞多。
     结论即刻种植即刻负重种植体使用PRF,可促进软组织的愈合,缩短软组织愈合的时间,有利于种植体周骨缺损区牙槽骨的保存与新骨的形成,促进种植体周围的骨结合。
Objective
     Through the establishment of animal model of immediate implant immediate loading implant, to observe the growth of soft and hard tissue with platelet-rich fibrin.
     Methods
     The bilateral mandible 4th premolar (PM4) of 5 beagle dogs was extracted. Immediately the implant surgery was operated. Left and right side of each animal mandible were implanted one titanium cylindrical screw threaded implant respectively. The implant superstructure was installed after implant surgery immediately in order to ensure immediate loading. Self-controlled design was applied in experiment. Experimental side was filled PRF in the buccal of implant, the interval defect and the tooth extraction socket, the buccal defect implant filler do not fill any filler, interval bone not prepared defect, tooth socket was filled with gelatin sponge. the second day after planting day the healing of soft tissue was observed; the planting day and 3 months after surgery, X-periapical films ware taken to observe the new bone around implant; 3 months after, the location and formation of new bone was observed, and measured and recorded. At the same time, bilateral buccal bone of implants, the interval bone and extraction socket bone were cut, and HE sections were made and observed under the light microscope.
     Results
     After planting, the observation of the average of soft tissue healing time were 5.80±0.84 days in the experimental group,8.20±0.45 days in the control group. Three months after planting, experimental X-ray films shows images denser than the control group. The average increased height of the buccal bone was 2.60±1.19mm in the experimental group, 1.60±1.52mm in the control side;the average increased buccal defect area was 1.45±1.46 mm2 in the experimental group,3.00±2.15 mm2 in the control side. Light microscopy of bone sections shows that the osteoblasts in the buccal of implant in experimental group more than in the control group; the maturity of osteoblasts in the interval bone was similar to the control group; the osteoblasts in the tooth extraction socket in experimental group more than in the control group.
     Conclusion
     Immediate implant immediate loading of implants with PRF, will promote the healing of soft tissue, reduce the healing time of soft tissue, promote the preservation of alveolar bone with new bone formation, promote the osseointegration.
