脑胶质瘤危险因素的病例对照研究

摘要
目的:探讨脑胶质瘤发病的危险因素,为脑胶质瘤的病因研究,及其一级预防提供理论依据。
     方法:采用1:1配比的病例对照研究,以2006年5月~2007年6月期间在兰州大学附属第二医院就诊,并经术后病理诊断确诊为脑胶质瘤的患者作为病例组,共103例(男性53例,女性50例),平均年龄37.10±5.35岁(12~65岁)。对照组来自兰州大学附属第二医院非患神经系统疾病及其他肿瘤性疾病的同期住院患者。按照1:1的配比,要求年龄上下不差3岁,同性别,同民族,同一或邻近居住地区,选取对照病例103例,平均年龄36.27±5.64岁(14~64岁)。两组调查对象在年龄、职业分布、学历水平、婚姻状况等方面无显著差异。采用由卫生部统一制定的专门问卷表,由经过统一培训的专业人员对调查对象进行面对面访谈式问卷调查,由专业统计人员对调查结果进行整理并统计,统计数据主要采用SPSS11.5进行分析,应用条件Logistic回归分析进行单因素及多因素分析。
     结果:1.单因素分析显示对照组与病例组在油炸食品摄入频次,喜食腌渍食品,吸烟,农药有机溶剂接触史,肿瘤家族史,5个因素为差异有统计学意义的因素,而在是否喜食腌渍食品,饮食习惯等方面无显著差异。2.多因素条件Logistic回归分析显示,农药的接触史,吸烟,肿瘤的家族史与胶质瘤的发病有密切关系。
     结论:农药接触史,吸烟,肿瘤的家族史是脑胶质瘤发病的危险因素。
Objective To explore the risk factors of glioma and to provide theory foundation for studying the etiology and primary prevention of glioma.
     Methods A 1∶1 matched case-control study method was adopted. All cases and controls were recruited from the Second Affiliated Hospital of Lanzhou University. 103 new glioma patients (male53, female 50)who visited our OPD from May 2006 to June 2007 and made glioma pathological diagnosis after sugery were selected as case group, average age 37.10±5. 35 (aged 12 to 65). other non-glioma patients were as control group, average age 36.27±5.64 (aged 14 to 64). there were not significant deviation in age , educational background , occupation and marriage et al between two groups. The individual questionnaire inventory were designed by MOH, which contained detailed content and relevant factors with glioma. Face-to-face investigation was carried out adopting the questionnaire. Conditional logistic regression analysis was used to develop for risk factors of glioma.
     Results One-way analysis showed the smoking, pesticides exposure , eating pickle, and family history of tumor, et al have relationship with glioma. Multivariate conditional logistic regression analysis showed pesticides exposure , smoking and family history of tumor were associated with glioma.
     Conclusion The pesticides exposure, smoking and family history of tumor were risk factors of glioma.
