基于现代分析技术的中药扶正平消胶囊质量控制研究

摘要
中药复方是在中医理论指导下,历代医家按照“君臣佐使”的组方原则,选择适宜的药味和剂量配伍而成的一类传统制剂。中药复方的有效性在临床应用中已经得到证实,中药复方用药已经被人们普遍接受。提高及完善中药复方制剂质量控制方法势在必行。
     肝脏肿瘤是我国最常见的恶性肿瘤之一,发病率及死亡率都很高。肝脏肿瘤的治疗方法很多,常见的有手术治疗,介入治疗,基因疗法等多种,虽然各种疗法都有其优势,但不可避免又都有着各自的缺陷,目前的治疗趋势是倾向于选择以某种疗法为主的综合性治疗方案,而在这种综合治疗当中,在改善患者症状、提高生存率方面,中医药治疗占了极其重要的地位。扶正平消胶囊是我院按照中医辨证施治原则,结合多年的临床治疗经验,精选二十八味药材组方而成,使用多年,效果较好,但由于其组方复杂,一直缺乏系统研究。新版《军队制剂规范》中规定,对中药复方制剂中至少三分之一成分进行定性定量分析。本课题选取该药,以中药现代化质量控制思想为指导,对其进行了全面系统的研究。
     采用HPLC-TOF/MS对扶正平消胶囊中的化学成分进行定性鉴别。根据国内外专业数据库及相关研究文献,建立扶正平消胶囊各药材的化学成分数据库。实验过程中分别采集了扶正平消胶囊提取物的正、负离子流图,利用飞行时间质谱进行精确分子量测定,比对其化学成分数据库,鉴别出其中266个化学成分,并对鉴别出的成分进行了药材归属,表征了扶正平消胶囊的化学物质基础,为其后续质量控制提供依据。
     在利用HPLC-TOF/MS对扶正平消胶囊中的化学成分进行定性鉴别基础上,选取其中的君药黄芪、吴茱萸、玄参、夏枯草、龙胆、桃仁,臣药浙贝母,佐药白芍,使药甘草等药材的标志性化合物作为含量测定对象,按其响应特点,分正、负离子两个检测模式。优化了提取条件,建立了HPLC-MS正离子检测模式下对于苦杏仁苷、芍药苷、贝母素甲、黄芪甲苷、吴茱萸次碱的含量测定方法;以及HPLC-MS负离子检测模式下对于龙胆苦苷、甘草苷、哈巴俄苷、迷迭香酸的含量测定方法。经方法学验证,两种方法均准确可靠,重复性好,回收率高,并成功应用于三个批次扶正平消胶囊中9个标志性化合物的含量测定,为全面控制扶正平消胶囊的质量提供有力的依据。对扶正平消胶囊药材中挥发油成分采用GC-MS进行分析鉴别与定性研究,由于GC-MS联用仪配有EI离子源,EI离子源所得质谱图具有重现性高,有相应的化学成分数据库等特点,可借助计算机检索与质谱数据库自动匹配进行鉴别分析,实验中共成功鉴别出扶正平消胶囊药材中73个成分,并对其中48个成分进行了药材归属,更全面的了解扶正平消胶囊药材中的化学成分,为其后续质量控制及临床药理相关研究提供基础。
     在对扶正平消胶囊化学成分分析鉴别的基础上,通过给SD大鼠灌胃给药扶正平消胶囊,借助HPLC-TOF/MS,对比体外扶正平消胶囊化学成分峰,对大鼠血浆中成分进行分析鉴别,共鉴别出大鼠血浆中16个以原型入血的成分。对扶正平消胶囊入血成分进行初步探索,为研究其可能的药效成分奠定了基础。
Chinese medicine prescription is a kind of Biotech drugs which made of appropriate medicinal odour and dosage, according to the "traassistant and guide" of principles of formulating prescription under the theory of traditional Chinese medicine by the ancient Chinese medicine doctors. The validity of the Chinese Medicine Prescription is proved in the clinical research. Now, Chinese medicine prescription is already widely accepted. Establishing the quality control method of the Chinese medicine prescripition is extremely important.
     Liver cancer is one of the most common malignant tumors in China with high morbidity and mortality. There are many strategies for liver tumor in clinical treatment, such as surgical treatment, interventional therapy, gene therapy and so on. Each strategy has its own advantages and disadvantages. In improving symptoms and survival of patients, Chinese medicine treatment plays an important role. In the guideline of TCM syndrome differentiation and clinical experience, Fuzhengpingxiao capsule is made of twenty-eight Chinese herbal medicines. It has been used for many years with good therapeutic effect. Because of its complicated components, there is not a systematic research for this medicine. According to Military Preparation Standard, it is necessary to qualitation and quantitation for as least one third components in a Chinese Medicine Prescription. We did a series of deeply and systematic research for Fuzhengpingxiao capsule.
     HPLC-TOF/MS was used the identification of constituents in Fuzhengpingxiao capsule. According to the professional databases and related literature, the database of constituents in Fuzhengpingxiao capsule was established. The mass response of capsule extraction was evaluated in both positive and negative ionization mode. After searching against the formula database established by ourselves,266 compounds were identified by HPLC-TOF/MS, and each constituent was indicated its resource herb medicine. The research characterized the chemical substance of Fuzhengpingxiao capsule and provided the instruction for its quality control.
     Based on the identification of constituents in Fuzhengpingxiao capsule by HPLC-TOF/MS, typical compounds in principal drug(Radix Astragali, Fructus Evodiae、Radix Scrophulariae、Spica Prunellae、Radix et Rhizoma Gentianae、Semen Persicae), ministerial drug(Bulbus Fritillariae Thunbergii), adjunctive drug(Radix Paeoniae Alba), meaaenger drug(Radix et Rhizoma Glycyrrhize) were selected to be quantitated. The extract conditions were optimized. A HPLC-(+)ESI/MS method for quantitation of amygdalin(Semen Persicae), paeoniflorin(Radix Paeoniae Alba), peimine(Bulbus Fritillariae Thunbergii), astragaloside IV(Radix Astragali), rutaecarpine(Fructus Evodiae), and a HPLC-(-)ESI/MS method for gentiopicroside(Radix et Rhizoma Gentianae), liquiritin(Radix et Rhizoma Glycyrrhize), rosmarinic acid(Spica Prunellae), harpagoside(Radix Scrophulariae) were established. The methods were successfully applied on determination of 9 compounds in 3 batches of Fuzhengpingxiao capsule. There were helpful in quality control of Fuzhengpingxiao capsule.
     GC-MS were used for the rapid identification of volatile components in Fuzhengpingxiao capsule.73 compounds were identified by GC-MS after searching against the formula database supplied by GC-MS instrument.48 compounds were indicated their resource herb medicine. The Fuzhengpingxiao capsule was also oral administrated to SD rats for investigate the migration components in serum.16 compounds were identified by HPLC-TOF/MS and database. It was only a preliminary study and provided hints to the possible medicinal composition of uzhengpingxiao capsule.
