摘要
目的探讨生理妊娠过程中及产后三酰甘油和胆固醇水平的变化。方法选择2017年3月至2018年1月于我院进行全程孕期检查和分娩的628名健康孕妇为观察组,另选择同期在我院进行健康体检的150名非妊娠健康女性作为对照组,分别测定并比较对照组及观察组孕妇孕早期、孕中期、孕晚期和产后的血清三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)水平。结果观察组孕早期与对照组血清TG、TC、HDL-C和LDL-C水平比较,无显著差异(P>0.05);观察组孕中期和孕晚期血清TG、TC、HDL-C和LDL-C水平显著高于孕早期和对照组,且孕晚期显著高于孕中期(P<0.05);产后血清TG、TC、HDL-C和LDL-C水平均显著低于孕晚期,但TG、TC和LDL-C水平仍显著高于孕早期、孕中期和对照组(P<0.05);产后与孕中期血清HDL-C水平比较,无显著差异(P>0.05)。结论生理妊娠过程中及产后孕妇血脂代谢改变明显,孕中、后期血清TG和胆固醇水平明显升高,产后逐渐降低。孕期和产后监测血脂变化有助于母婴健康。
        Objective To investigate the changes of levels of triglyceride and cholesterol during physiological pregnancy and postpartum. Methods A total of 628 healthy pregnant women who underwent full pregnancy examination and delivery in our hospital from March 2017 to January 2018 were selected as observation group. Another 150 non-pregnant healthy women in our hospital for health checkups in the same period were selected as control group. The levels of serum triglyceride(TG), total cholesterol(TC), high-density lipoprotein cholesterol(HDL-C) and low density lipoprotein cholesterol(LDL-C) in the control group and the observation group at the first trimester, the second trimester, the third trimester and the postpartum were measured and compared. Results There were no significant differences in the levels of serum TG, TC, HDL-C and LDL-C between the observation group at the first trimester and the control group(P>0.05); the levels of serum TG, TC, HDL-C and LDL-C in the observation group at the second trimester and the third trimester were significantly higher than those at the first trimester and the control group, and those at the third trimester were significantly higher than the second trimester(P<0.05); the levels of serum TG, TC, HDL-C and LDL-C at postpartum were significantly lower than those at the third trimester, but the levels of TG, TC and LDL-C were still significantly higher than those at the first trimester, the second trimester and the control group(P <0.05). There was no significant difference in the levels of serum HDL-C at the second trimester and postpartum(P>0.05). Conclusion During the process of physiological pregnancy and postpartum, the blood lipid metabolism of pregnant women changed significantly. The levels of serum TG and cholesterol increased significantly during the second and the third trimester, and gradually decreased after delivery.Monitoring blood lipid during pregnancy and postpartum is helpful to maternal and infant health.
