肝脏上皮样血管内皮瘤影像表现、病理对照及文献复习

摘要
目的:通过对肝脏上皮样血管内皮瘤的影像学表现及病理表现进行研究,回顾相关文献,以进一步认识该疾病的影像学特征。方法:对7例经手术或穿刺活检病理证实的肝脏上皮样血管内皮瘤进行回顾性分析。结果:(1)形态改变:7例均为多发型,4例结节状生长方式、2例肿块状生长方式、1例匍匐生长方式;(2)密度/信号及强化特点:7例CT均呈低密度,MRI呈T1WI稍低信号、T2WI稍高信号。弥散序列呈稍高信号。6例增强检查中4例见"棒棒糖征"。结论:HEHE的影像学形态特点、密度/信号及强化特点对于肝脏上皮样血管内皮瘤的术前诊断有重要意义。
Objective: To study the imaging and pathological features of hepatic epithelioid hemangioendothelioma and review the relevant literature to further understand the imaging features of the disease. Methods: Retrospective analysis was performed on 7 hepatic epithelioid hemangioendothelioma cases confirmed by surgery or biopsy. RESULTS:( 1) Morphological changes: All the 7 cases had multiple hair styles,4 nodular growth patterns,2 mass growth patterns,and 1 growth pattern;( 2) Density/signal and enhancement characteristics: 7 cases showed low density in CT imaging and the signal intensity was slightly lower on T1 WI and slightly higher on T2 WI in MRI imaging. The diffuse sequence showed a slightly higher signal. Of 6 cases of enhanced scanning, " lollipop sign" was seen in 4 cases.Conclusion: The imaging features,density/signal and enhancement features of HEHE are important for the preoperative diagnosis of hepatic epithelioid hemangioendothelioma.
