严世芸治疗心律失常用药规律研究

摘要
目的分析严世芸教授治疗心律失常处方用药特点和配伍规律,为临床治疗提供参考。方法收集严世芸教授2014年8月-2016年12月在上海中医药大学附属曙光医院、岳阳中西医结合医院、上海市中医医院及上海市中医文献馆门诊治疗的心律失常病案。采用中医传承辅助平台(V2.5)对纳入处方进行药物频数、配伍规律、核心药物组合等分析。结果纳入处方203首,涉及药物167味,高频药物有川芎(184次)、桃仁(172次)、黄芪(167次)、地龙(167次)、土鳖虫(160次)等;药物性味以温(2549次)、平(2068次)、寒(1283次)性及甘(2591次)、苦(1809次)、辛(1573次)味为主;药物归经以足厥阴肝经(1221次)、足太阴脾经(1027次)及手少阴心经(975次)为主。高频药对有"川芎-桃仁""川芎-土鳖虫""川芎-地龙"等,挖掘得到关联规则25条,提取新处方6首。结论严世芸教授治疗心率失常善用益气活血化瘀药物,遣方用药特点鲜明、主次兼顾,可为该病治疗提供参考。
Objective To analyze Professor YAN Shi-yun's medication characteristics and compatibility principles of prescriptions for arrhythmia; To provide references for clinical treatment. Methods Arrhythmia cases treated by Prof. YAN Shi-yun from August 2014 to December 2016 in Shuguang Hospital affiliated to Shanghai University of Traditional Chinese Medicine, Yueyang Hospital of Integrated Traditional Chinese and Western Medicine, Shanghai Traditional Chinese Medicine Hospital and Shanghai Traditional Chinese Medicine Literature Museum were collected. Medicine frequency, compatibility principles, and core medicine combinations of included prescriptions were analyzed by using TCM Inheritance Support Platform System(V2.5). Results Totally 203 prescriptions containing 167 medicines were collected. Medicines with high frequency were Chuanxiong Rhizoma(184 times), Persicae Semen(172 times), Astragali Radix(167 times), Pheretima(167 times) and Eupolyphaga Steleophaga(160 times). The main medicine properties were warm(2549 times), neutral(2068 times) and cold(1283 times), and the main medicine tastes were sweet(2591 times), bitter(1809 times) and spicy(1573 times). Medicine efficacy in meridians was mainly in Liver Meridian of Foot-Jueyin(1221 times), Spleen Meridian of Foot-Taiyin(1027 times), and Heart Meridian of Hand-Shaoyin(975 times). The medicine pairs with high frequency were "Chuanxiong Rhizoma-Persicae Semen", "Chuanxiong Rhizoma-Phertima", and "Chuanxiong Rhizoma-Eupolyphaga Steleophaga". 25 rules of association were obtained by mining, and 6 new prescriptions were extracted. Conclusion Prof. YAN Shi-yun's treatment for arrhythmia makes good use of medicines which can tonify qi and invigorate the circulation of blood. The medication characteristics are unique, balancing the primary and secondary, which can provide references for the treatment of arrhythmia.
