经皮氧和二氧化碳分压监测值与动脉血氧和二氧化碳分压的相关性研究

摘要
目的探讨经皮氧(P_(TC)O_2)和经皮二氧化碳分压(P_(TC)CO_2)和动脉血氧(PaO_2)和二氧化碳分压(PaCO_2)相关性。方法选取2016年7月—2017年10月深圳市南山区人民医院收治的34例重症颅脑外伤患者作为研究对象,患者入院时(T0)、入院1 h(T1)、入院4 h(T2)、入院8 h(T3)、入院12 h(T4)抽取动脉血检测PaO_2、PaCO_2水平,同时在相同时间节点检测P_(TC)O_2、P_(TC)CO_2水平,采用线性回归和Pearson分析两种检测方法相关性。结果 T0、T1、T2、T3、T4时间节点,P_(TC)O_2水平均低于PaO_2水平,P_(TC)CO_2水平均低于PaCO_2水平。线性回顾分析得出,P_(TC)O_2、P_(TC)CO_2水平和PaO_2、PaCO_2水平在各个时间节点均有良好相关性,TCO_2和PaO_2相关性:P_(TC)O_2=-10.1+0.98×PaO_2;P_(TC)CO_2和PaCO_2相关性:P_(TC)CO_2=23.5+0.31×PaCO_2。Pearson分析得出,P_(TC)O_2和PaO_2水平呈正相关性(r=0.955,P=0.000);P_(TC)CO_2和PaCO_2水平呈正相关性(r=0.698,P=0.000)。结论 P_(TC)O_2和P_(TC)CO_2与PaO_2和PaCO_2有良好相关性,对于动态监测PaO_2和PaCO_2水平患者,可用P_(TC)O_2和P_(TC)CO_2替代。
        Objective To investigate the correlation between percutaneous oxygen( P_(TC)O_2) and partial pressure of carbon dioxide( P_(TC)CO_2) and arterial oxygen( PaO_2) and partial pressure of carbon dioxide( PaCO_2). Method Selecting 34 severe craniocerebral trauma patients in the hospital from Jul. 2016 to Oct. 2017 as study object,PaO_2 and PaCO_2 levels were measured at admission( T0),admission 1 h( T1),admission 4 h( T2),admission 8 h( T3) and admission 12 h( T4),meanwhile P_(TC)O_2 and P_(TC)CO_2 were detected at the same time point. The relevance of the two detection methods of linear regression and Pearson was analyzed. Result At the time points of T0,T1,T2,T3 and T4,P_(TC)O_2 levels were lower than those of PaO_2,and P_(TC)CO_2 levels were lower than that of PaCO_2. Linear regression analysis showed that P_(TC)O_2,P_(TC)CO_2 levels and PaO_2,PaCO_2 levels at all time points were of good correlation,P_(TC)O_2 and PaO_2 correlation was P_(TC)O_2=-10. 1 + 0. 98 PaO_2; P_(TC)CO_2 and PaCO_2 correlation was P_(TC)CO_2= 23. 5 + 0. 31 ×PaCO_2. Pearson analysis showed that P_(TC)O_2 and PaO_2 levels were positively correlated( r = 0. 955,P = 0. 000); P_(TC)CO_2 and PaCO_2 levels were positively correlated( r = 0. 698,P = 0. 000). Conclusion P_(TC)O_2 and P_(TC)CO_2 have a good correlation with PaO_2 and PaCO_2,and can be replaced with P_(TC)O_2 and P_(TC)CO_2 for patients with dynamic monitoring of PaO_2 and PaCO_2 levels.
