降低初产妇会阴侧切率对新生儿窒息的影响研究

摘要
目的:研究降低初产妇会阴侧切率对新生儿窒息的影响。方法:筛选2016年1月～2017年12月某院妇产科收治的366例初产妇作为研究对象,按照是否行会阴侧切术分为A组(未行会阴侧切360例)和B组(行会阴侧切6例),对比分析两组产妇在会阴侧切例数、会阴侧切率、切口感染率、中度裂伤情况及新生儿窒息率。结果:除却两组产妇中度裂伤无明显差异(P>0.05),A组产妇在伤口愈合不良方面优于B组,组间对比差异显著(P<0.05);A组新生儿窒息率为1.38%,B组新生儿窒息率为16.66%,经统计学对比分析有意义(P<0.05)。结论:降低初产妇会阴侧切率能够有效降低会阴感染率和新生儿的窒息率,但对会阴中度裂伤基本无影响,临床中可通过提高接生和助产技术水平,减少会阴侧切的可能。
Objective:To study the effect of reducing perineum side cut rate on neonatal asphyxia.Methods:366 cases of primipara admitted in a hospital from January 2016 to December 2017 obstetrics were selected as the research object and divided into A group(without receiving lateral resection in 360 cases)and B group(receiving lateral resection in 6 cases)according to whether the perineal side incision,maternal episiotomy cases episiotomy rate,wound infection rate,moderate lacerations and neonatal asphyxia of two groups were compared and analyzed.Results:There was no significant difference between the two groups in moderate laceration(P>0.05),but the parturients in group A were superior to group B in poor wound healing,the difference between two groups was statistically significant(P<0.05).The rate of neonatal asphyxia in group A was significantly higher than that in group B(P<0.05),and the rate of neonatal asphyxia in group A was1.38%,in group B was 16.66%.There was significant difference between the two groups(P<0.05).Conclusion:The reduction of perineal side incision rate can effectively reduce perineal infection rate and neonatal asphyxia rate,but it has no effect on perineal moderate laceration.The possibility of perineal lateral resection can be reduced by improving the technical level of delivery and midwifery in clinic.
