豁痰化瘀利水方联合西药对创伤性脑损伤的疗效观察

摘要
目的比较豁痰化瘀利水方联合西药对创伤性颅脑损伤的疗效的分析,探讨创伤性颅脑损伤临床中医治疗特点,提高中西医结合在临床诊治中的水平。方法收集西藏民族大学附属医院2015年7月至2017年10月收治的急性创伤性脑损伤的相关临床病例及随访病例资料。对住院患者均进行头颅CT扫描检查,根据格拉斯哥昏迷计分法(GCS)分级进行系统评估,将110例创伤性脑损伤的患者随机分为治疗组和对照组各55例。在常规治疗的基础上,对照组给予单纯西药常规治疗,治疗组在常规西药治疗的基础上加用豁痰化瘀利水方治疗,疗程为24天,其他治疗与实验组相同,观察两组治疗的疗效差别。结果经过脱水治疗,抗感染,保持呼吸道通畅,高压氧和康复治疗。治疗组总有效率为93.4%,对照组总有率为64.7%,治疗组疗效明显优于对照组,且差异有统计学意义(P<0.05)。结论豁痰化瘀利水方能明显减轻创伤性脑损伤,且可有效减轻脑损伤的继发性脑改变,明显改善脑损伤预后,降低再出血的危险。
Objective To observe the clinical data of traumatic brain injury treated by Huotan Huayu Lishui recipe combined with Western medicine, and to explore the clinical treatment characteristics of traumatic brain injury and improve its clinical diagnosis and treatment level. Methods The clinical and follow-up data of acute traumatic brain injury admitted to the Affiliated Hospital of Tibet University for Nationalities from July 2015 to October 2017 were collected. All the inpatients were examined by cranial CT scans and evaluated according to different grades. 110 patients with traumatic brain injury were randomLy divided into treatment group and control group with 55 cases in each group. On the basis of routine treatment, the control group was treated with routine western medicine only, while the treatment group was treated with Huotan Huayu Lishui Decoction on the basis of routine western medicine.The course of treatment was 24 days. The other treatments were the same as the experimental group. The difference of therapeutic effect between the two groups was observed. Results After dehydration, anti-infection, airway patency, hyperbaric oxygen and rehabilitation treatment were maintained. The total effective rate was 93.4% in the treatment group and 64.7% in the control group. The curative effect of the treatment group was significantly better than that of the control group, and the difference was statistically significant(P<0.05).Conclusion Huotan Huayu Lishui Decoction can significantly reduce traumatic brain injury, and can effectively alleviate secondary brain changes of brain injury, significantly improve the prognosis of brain injury and reduce the risk of re-hemorrhage.
