内镜黏膜下剥离术在早期胃癌治疗中的应用效果

摘要
目的探究内镜黏膜下剥离术在早期胃癌治疗中的应用效果。方法随机选取2012年1月至2015年1月在我院治疗的50例早期胃癌患者为研究对象,根据患者意愿将其分为试验组和对照组,每组25例。试验组给予内镜黏膜下剥离术治疗,对照组给予外科手术治疗,比较两组治疗效果。结果试验组并发症总发生率显著低于对照组(P<0.05);两组术后复发率及术后3年生存率比较,差异无统计学意义(P>0.05);试验组手术时间长于对照组,住院时间短于对照组,住院费用低于对照组(P<0.05);试验组一次性整块切除率、一次性完整切除率、组织学完整治愈切除率均高于对照组(P<0.05)。结论内镜黏膜下剥离术治疗早期胃癌的效果与外科手术相似,但在住院时间、住院费用、并发症发生情况等方面更具优势,临床上可结合患者病情与经济能力为其提供治疗方案。
Objective To explore the application effect of endoscopic submucosal dissection in the treatment of early gastric cancer. Methods A total of 50 patients with early gastric cancer treated in our hospital from January 2012 to January 2015 were randomly selected as the research objects and divided into experimental group and control group according to patients' wishes, with 25 cases in each group. The experimental group was treated with endoscopic submucosal dissection, while the control group was treated with surgery. The treatment effects between the two groups were compared.Results The total incidence of complications in the experimental group was significantly lower than that in the control group(P<0.05). There were no significant differences in the postoperative recurrence rate and 3-year survival rate between the two groups(P >0.05). The operation time of the experimental group was longer than that of the control group, the hospitalization time was shorter than that of the control group, and the hospitalization expense was lower than that of the control group(P<0.05). The rates of one-time whole block resection, one-time complete resection and histological complete cure resection in the experimental group were all higher than those in the control group(P<0.05). Conclusion Endoscopic submucosal dissection in the treatment of early gastric cancer has similar effect with the surgery, but it has more advantages in hospitalization time, hospitalization expenses, complications and other aspects. It can provide treatment programs based on patients' condition and economic ability in clinic.
