牙种植修复在慢性牙周炎患者与牙周健康者的疗效及预后分析

摘要
目的分析牙种植修复在慢性牙周炎患者与牙周健康者的疗效及预后。方法选择我院2013年12月至2018年5月进行牙种植修复的63例患者,其中32例(37枚)慢性牙周炎患者为研究组, 31例(35枚)牙周健康者为对照组。比较两组的修复效果与预后情况。结果修复后6个月、 1年,两组的种植成功率比较无统计学差异(P>0.05);修复后3年、 5年,研究组的种植成功率均低于对照组(P <0.05)。研究组修复后的PIS、 MSBI、 PD均高于对照组(P <0.05)。研究组修复后1年、 3年、 5年的牙槽骨吸收分别为(1.37±0.23) mm、(1.56±0.34) mm、(1.64±0.42) mm,均高于对照组的(0.37±0.10) mm、(0.43±0.12) mm、(0.54±0.13) mm (P <0.05)。结论牙种植修复对牙周健康者与慢性牙周炎患者的近期疗效无明显差异,但慢性牙周炎患者的远期疗效较差,主要是因为慢性牙周炎致使牙槽骨吸收增加。
Objective To analyze the curative effect and prognosis of dental implant restoration in patients with chronic periodontitis and periodontal health. Methods 63 cases of patients undergoing dental implant restoration in our hospital from December 2013 to May2018 were selected. 32 patients(37 pieces) with chronic periodontitis were selected as study group, and 31 patients(35 pieces) with periodontal health were selected as control group. The restoration effect and prognosis of the two groups were compared. Results 6 months and 1 year after restoration, no statistical difference was found in the success rates of implantation between the two groups(P >0.05). 3 years and 5 years after restoration, the success rates of implantation of the study group were lower than those of the control group(P <0.05).After restoration, the PIS, MSBI and PD of the study group were higher than those of the control group(P <0.05). 1 year, 3 years and 5 years after restoration, the alveolar bone resorption of the study group were respectively(1.37 ± 0.23) mm,(1.56 ± 0.34) mm and(1.64 ± 0.42)mm, higher than(0.37 ± 0.10) mm,(0.43 ± 0.12) mm and(0.54 ± 0.13) mm of the control group(P <0.05). Conclusions No significant difference is found in the short-term curative effect of dental implant restoration on patients with chronic periodontitis and periodontal health.However, patients with chronic periodontitis have worse long-term curative effect, mainly due to the increase of alveolar bone resorption caused by chronic periodontitis.
