摘要
目的:探讨128排256层螺旋计算机断层扫描(CT)血管造影术在评估经皮冠状动脉介入术(PCI)患者支架内再狭窄(ISR)中的应用。方法:选取2017年7月至2018年2月秦皇岛市第一医院PCI患者148例,共190枚支架,依据ISR发生情况分为ISR组(n=24枚)和无ISR组(n=166枚),给予128排256层螺旋CT血管造影术检查,以冠脉造影(CAG)为对照。结果:以CAG为对照,CT血管造影术评估ISR的敏感度、特异度、准确度分别为91.67%、92.77%、92.63%,差异无统计学意义(P>0.05);在不同血管部位成像质量评分及评估符合率方面,左主干(LM)、前降支(LAD)明显高于回旋支(LCX)、右冠状动脉(RCA),差异有统计学意义(P<0.05)。结论:128排256层螺旋CT血管造影术对PCI患者ISR具有良好的评估价值,尤其在LM、LAD血管部位的评估中具有更佳的成像质量及评估符合率。
        Objective: To discuss the application of 128 row 256 slice spiral computed tomography(CT) angiography in evaluating instent restenosis(ISR) of patients with percutaneous coronary intervention(PCI). Methods: 148 patients with PCI were selected from July 2017 to February 2018 in our Hospital, a total of 190 stents, according to the occurrence of ISR, all patients were divided into ISR group(n =24 cases) and non ISR group(n =166 cases), they were given 128 rows of 256 slice spiral CT angiography, the coronary angiography(CAG) as the control. Results: Compared with CAG, the sensitivity, specificity and accuracy were respectively of CT angiography evaluating the ISR were 91.67%, 92.77%, 92.63%, the difference was not statistically significant(P>0.05). In the vascular imaging quality score and coincidence rate in different parts, the left main(LM), left anterior descending(LAD) were significantly higher than those of the left circumflex(LCX), right coronary artery(RCA), the difference was statistically significant(P <0.05). Conclusion: 128 row and 256 slice spiral CT angiography has good value in evaluating ISR in patients with PCI, especially in LM and LAD vascular location evaluation.
