摘要
目的分析2014~2015年广西钦州市新报告HIV/AIDS病例HIV1型(HIV-1)分子传播网络特征。方法 2014~2015年在广西钦州市招募新报告且未治疗的355例HIV/AIDS患者为研究对象,进行流行病学调查。提取所有研究对象外周血中HIV-1 RNA进行扩增和测序,分析HIV-1流行重组亚型。基于最适基因距离构建分子传播网络,并分析入网率及其影响因素。结果共获得264条pol区序列,主要亚型为CRF08_BC(52.27%),其次为CRF01_AE(39.77%),再次为CRF07_BC(6.06%)。CRF01_AE亚型的主要传播途径为异性性传播(84.76%),CRF07_BC和CRF08_BC亚型的主要传播途径为异性性传播和注射吸毒传播。在1.65%最适基因距离阈值下,构建HIV-1分子传播网络,总共有154条序列入网,总入网率为58.33%; CRF01_AE、CRF07_BC、CRF08_BC彼此独立成簇。性别、年龄、民族、职业、传播途径、CD4~+细胞计数和亚型均非HIV-1进入分子传播网络的影响因素(均P>0.05)。分子传播网络中不同HIV-1基因亚型的关联程度的分布差异有统计学意义(P<0.05),其中CRF08_BC的关联程度均高于其他亚型。结论广西钦州市新报告未治疗HIV/AIDS人群的HIV-1流行亚型以CRF08_BC为主。应构建分子传播网络对该地区CRF08_BC亚型进行长期监测,以采取针对性的措施阻断其传播。
        Objective To analyze the characteristics of HIV type 1(HIV-1) molecular transmission network of newly reported HIV/AIDS cases in Qinzhou City of Guangxi from 2014 to 2015.Methods From 2014 to 2015,355 newly reported and treatment-naive HIV/AIDS patients were recruited as subjects in Qinzhou City of Guangxi,then an epidemiological survey was conducted. HIV-1 RNA was extracted from the peripheral blood of all subjects for amplification and sequencing,then predominant recombinant subtype of HIV-1 was analyzed. Molecular transmission network was constructed based on the optimal gene distance,then accession rate and its influencing factors were analyzed. Results Among 264 pol sequences obtained,the predominant subtype was CRF08 _ BC(52.27%),followed by CRF01 _ AE(39.77%) and CRF07_BC(6.06%). Subtype CRF01 _AE infection was mainly mediated by heterosexual contact(84.76%),while subtype CRF07_BC or CRF08_BC infection mainly by heterosexual contact and drug injection. The HIV-1 molecular transmission network was constructed based on the optimal gene distance threshold of 1.65%,and the network consisted of 154 sequences with the total accession rate of 58.33%; CRF01_AE,CRF07_BC and CRF08_BC were clustered independently.Gender,age,nationality,occupation,route of transmission,count of CD4~+ cell,and subtype were not the influencing factors for accession of HIV-1 to molecular transmission network(all P>0.05). There was statistically significant difference in distribution of relevance among different subtypes of HIV-1 gene in the molecular transmission network(P<0.05),and the relevance of CRF08_BC was higher than that of the other subtypes. Conclusion In Qinzhou City of Guangxi,CRF08_BC was the predominant subtype in the newly reported and treatment-naive HIV/AIDS individuals.A molecular transmission network should be constructed to monitor subtype CRF08_BC in this region for a long time,so that specific measures can be adopted to prevent the transmission of CRF08_BC.
