摘要
目的探讨零回波时间技术磁共振血管成像(zTE-MRA)对头颈部动脉显示效果及其临床应用价值。方法对47例行头颈三维增强(CE)MRA检查的患者先行三维时间飞越法(3D TOF)MRA与zTE-MRA检查,对比分析3D TOF-MRA与zTE-MRA图像质量及颈内动脉分支显示情况;以CE-MRA所得血管像为标准,比较3D TOF-MRA与zTE-MRA两种非对比剂血管成像方法对头颈部动脉血管性病变显示的敏感度及特异度,评估两种不同技术的诊断准确率。结果 zTE-MRA技术血管成像的图像质量明显优于3D TOF-MRA,差异具有统计学意义(P<0.05),两种技术颈内动脉分支显示情况评分差异无统计学意义(P>0.05)。以CE-MRA为标准,zTE-MRA技术诊断敏感性及特异度均优于3D TOF-MRA技术。结论 zTE-MRA图像质量优于3D TOF-MRA,且诊断准确率明显高于3D TOF-MRA,在头颈部动脉成像时可替代CE-MRA,具有较好的临床应用价值。
Objective To investigate the image quality and clinical application value of zero echo time magnetic resonance angiography(zTE-MRA) on head-neck. Methods 47 patients assigned to head-neck contrast enhanced MRA(CE-MRA) were prospectively collected. Three-dimensional time-of-flight MRA(3 D TOF-MRA) and zTE-MRA were firstly and secondly performed in these patients before CE-MRA examination. Image quality and display of internal carotid artery branches were analyzed between 3 D TOF-MRA and zTE-MRA. And the diagnostic accuracy was evaluated through comparing the sensitivity and specificity of the two techniques with CE-MRA resulted as the diagnostic standard. Results The image quality of zTE-MRA was significantly better than that of 3 D TOF-MRA,and the differences were statistically significant(P<0.05). There were no significant differences in the scores of display of internal carotid artery branches between the two techniques(P>0.05). Using CE-MRA results as the standard,the sensitivity and specificity of zTE-MRA were superior to 3 D TOF-MRA in diagnostic accuracy. Conclusion zTE-MRA is better than 3 D TOF-MRA in image quality and higher in diagnostic accuracy. Consequently,zTE-MRA can serve as an alternative technique for CE-MRA on head-neck,and endow with better clinical practice value.
