研究超声心动图测定心外膜脂肪厚度在高血压性心脏病诊断中的价值

摘要
目的探讨心外膜脂肪厚度超声心动图检测诊断高血压性心脏病的临床价值。方法方便选取2017年1月—2018年12月该院76例高血压性心脏病患者为实验组,择取同期76例健康志愿者为对照组,超声心动图测定两组受检者心外膜脂肪厚度,比较观察两组以及实验组不同病变程度患者的指标水平差异。结果实验组左心室收缩末期心外膜脂肪厚度(7.16±1.52)mm,舒张末期心外膜脂肪厚度(3.82±0.96)mm,均高于对照组(t=5.172,3.967;P<0.05)。实验组轻、中、重度患者收缩末期EAT厚度分别为(6.03±1.95)mm、(8.37±2.26)mm、(9.45±2.73)mm,舒张末期EAT厚度分别为(3.28±0.77)mm、(4.33±0.91)mm、(5.12±1.04)mm,Person相关性分析示两者呈正相关(r=0.815,0.836;P=0.008)。结论超声心动图测定心外膜脂肪厚度能够为高血压性心脏病临床诊断提供有价值的依据,在评估患者病情严重程度方面也具有一定参考价值,值得临床推广使用。
        Objective To investigate the clinical value of epicardial fat thickness echocardiography in the diagnosis of hypertensive heart disease. Methods From January 2017 to December 2018, 76 patients with hypertensive heart disease in our hospital were convenient used as experimental group. 76 healthy volunteers in the same period were selected as the control group. Echocardiography was used to measure the epicardium of the two groups. The thickness of the fat was compared between the two groups and the difference in the index level of the patients with different degrees of disease in the experimental group. Results The left ventricular endocardial fat thickness(7.16±1.52) mm and the end-diastolic epicardial fat thickness(3.82±0.96) mm were higher in the experimental group than in the control group(t=5.172,3.967; P<0.05). The EAT thickness of the end-systolic phase of the experimental group was(6.03 ±1.95) mm,(8.37±2.26) mm,(9.45±2.73) mm, and the end-diastolic EAT thickness was(3.28±0.77) mm,(4.33±0.91) mm,(5.12±1.04) mm, Person correlation analysis showed a positive correlation(r=0.815, 0.836; P=0.008). Conclusion Echocardiographic determination of epicardial fat thickness can provide a valuable basis for the clinical diagnosis of hypertensive heart disease. It also has certain reference value in assessing the severity of the patient's condition. It is worthy of clinical promotion.
