重度窒息新生儿早期微量喂养联合呼吸机治疗的临床价值分析

摘要
目的评价重度窒息新生儿早期微量喂养联合呼吸机的治疗效果,评定临床价值。方法方便选取该院2016年4月—2017年4月新生儿重症监护室所收治的重度窒息新生儿60例,采用随机数表法将其平均分为实验组与对照组,每组30例。两组患儿均予以呼吸机进行治疗,对照组加行常规喂养法治疗,实验组加行早期喂养法治疗。对比分析两组患儿的肠胃激素、血气分析指标、治疗时间与并发症等数据。结果两组患儿的血气分析指标均得到明显改善,实验组血气指标中pH(7.37±0.1)mmol/L、血氧分压(11.0±0.4)mmHg、血二氧化碳分压(5.9±0.4)mmHg。对照组血气指标中pH(7.31±0.11)mmol/L、血氧分压(8.2±0.3)mmHg、血二氧化碳分压(5.7±0.3)mmHg(t=4.210 6、30.672 4、4.190 8,P=0.031 0、0.000 0、0.032 5)。实验组患儿的改善程度高于对照组。结论重度窒息新生儿治疗过程中,早期微量喂养联合呼吸机治疗具有明显效果,可改善患儿的胃肠道功能,并减少并发症,提升临床应用安全性,值得进行推广。
        Objective To evaluate the clinical value of early minimal feeding combined with ventilator therapy of newborns with severe asphyxia. Methods 60 cases of newborns with severe asphyxia admitted and treated in ICU in our hospital from April 2016 to April 2017 were selected and randomly divided into two groups with 30 cases in each, both groups were treated with ventilator, the control group added the routine feeding method, while the experimental group added the early feeding method, and the gastrointestinal hormones, blood gas analysis indexes, treatment time and complications were compared and analyzed. Results The blood gas analysis indicators of the two groups were improved obviously, and the pH(7.37±0.1) mmol/L, blood oxygen partial pressure(11±0.4) mmHg, blood carbon dioxide partial pressure(5.9±0.4) mmHg, were in the experimental group, and PH(7.31±0.11) mmol/L, blood oxygen partial pressure(8.2±0.3) mmHg, blood carbon dioxide partial pressure(5.7±0.3) mmHg were in the control group(t=4.210 6, 30.672 4, 4.190 8, P= 0.031 0, 0.000 0, 0.032 5),and the improvement degree of children in the experimental group was higher than that in the control group. Conclusion The effect of early minimal feeding combined with ventilator therapy of newborns with severe asphyxia is obvious, which can improve the gastrointestinal function of children, reduce the complications and improve the clinical application safety, and it is worth promotion.
