慢性心力衰竭患者红细胞压积的变化及临床意义

摘要
目的探讨慢性心力衰竭患者红细胞压积的变化及临床意义。方法选取2017年1月～2018年5月在本院收治并开展治疗的慢性心力衰竭患者60例,将其设定为研究组,再选择同期心功能正常的60例患者,将其设定为对照组,比较两组患者红细胞压积的差异性,并比较研究组治疗前后红细胞压积(VPRC)、LVEF和6 min步行试验的数值的变化。结果研究组VPRC为(39.0±7.3)%,对照组为(41.1±6.2)%,研究组较对照组低,两组患者之间红细胞积压存在显著差异(P<0.05),治疗后VPRC(40.1±7.0)%、治疗后LVEF为(47.0±13.1)%,治疗后6 min步行试验为(339.2±75.0)m,研究组治疗前后红细胞压积存在显著差异(P<0.05)。治疗后VPRC、LVEF明显升高,6 min步行试验的数值也显著升高,差异有统计学意义(P<0.05)。结论慢性心力衰竭患者中,其血红细胞压积较心功能正常患者低,血红细胞积压低有利于改善患者的新陈代谢和血管微循环灌注量,建议临床上进一步进行探究,其治疗后的VPRC、LVEF和6 min步行试验的数值的变化显著。患者心功能的改善及患者红细胞压积增加,对指导心衰的治疗和预后的判断有重要的临床意义。
Objective To investigate changes of hematocrit in patients with chronic heart failure and its clinical significance. Methods A total of 60 patients with chronic heart failure who treated in our hospital from January 2017 to May2018 were enrolled in the study group. 60 patients with normal cardiac function at the same period were selected and set as the control group. The differences in hematocrit between the two groups were compared. The changes in the values of VPRC, LVEF, and 6-minute walk test before and after treatment were compared in the study group. Results The VPRC of the study group was(39.0±7.3)%, which was lower than that of the control group(41.1±6.2)%. There was a significant difference in the hematocrit between the two groups(P<0.05). After treatment, the VPRC and LVEF were(40.1±7.0)%,(47.0±13.1)% respectively, and the 6-minute walking test was(339.2±75.0) m in the study group. There was significant difference in hematocrit before and after treatment in the study group(P<0.05). The VPRC and LVEF were significantly increased after treatment, the value of the 6-minute walking test was also significantly increased, and the difference was statistically significant(P<0.05). Conclusion The hematocrit in patients with chronic heart failure is lower than that of patients with normal heart function. The low hematocrit is beneficial to improve the metabolism and vascular microcirculation perfusion, which is recommended for further exploration in the clinical. The values of the VPRC, LVEF, and 6-minute walk tests after treatment were significantly changed. As the patients' cardiac function improves, the patients' hematocrit increases, and this change has important clinical significance in guiding the treatment and prognosis of heart failure.
