血清胃蛋白酶原Ⅰ、Ⅱ及其比值联合Hp抗体诊断老年早期胃癌

摘要
目的探讨血清胃蛋白酶原(PG)Ⅰ、Ⅱ及其比值联合幽门螺杆菌(Hp)抗体诊断老年早期胃癌的价值。方法老年早期胃癌患者284例作为胃癌组;同期120例慢性萎缩性胃炎患者为胃炎组和104例健康体检者为对照组。采用乳胶增强免疫比浊法对比3组血清PGⅠ、PGⅡ水平,酶联免疫吸附试验对比Hp抗体的表达,并分析PGⅠ和PGⅡ与Hp阳性率相关性。结果胃癌组血清PGⅠ和PGⅡ水平均显著低于胃炎组及对照组(P<0.05);胃炎组血清PGⅠ和PGⅡ水平显著低于对照组(P<0.05);3组PGⅠ/PGⅡ比值差异无统计学意义(P>0.05)。胃癌组Hp阳性率显著高于胃炎组和对照组(P<0.05),胃炎组Hp阳性率显著高于对照组(P<0.05)。PGⅠ和PGⅡ与Hp阳性率均呈显著负相关(r=-0.372 1、-0.689,P<0.05)。结论 PGⅠ、Ⅱ及比值联合Hp抗体可为老年早期胃癌临床诊断提供参考。
