儿童睾丸畸胎瘤的超声声像图特征分析

摘要
目的:探讨超声在儿童睾丸畸胎瘤诊断中的声像图特征及诊断价值。方法:回顾福建省厦门市妇幼保健院收治的17例儿童睾丸畸胎瘤,分析其临床特点、二维声像图和彩色多普勒表现。结果:17例患儿术前均行高频彩色多普勒超声检查。患儿因睾丸畸胎瘤声像图表现多样:体积大小不等,内呈囊性、囊实性或不均质实性回声。所有患儿均行手术剥除或切除术,术后病理检查结果显示,未成熟4例,成熟11例,恶性2例。良性睾丸畸胎瘤边界清晰,内部及周边无或偶见点状血流信号;而恶性畸胎瘤边界模糊,其内及周边可见丰富杂乱的粗条状血流信号。结论:儿童睾丸畸胎瘤超声声像图表现多样,同时表现有较高的特异性。可将超声作为临床辅助诊断睾丸畸胎瘤良恶性的重要方法。
Objective:To explore the features and diagnostic value of ultrasonography in the diagnosis of testicular teratoma in children.Method:Seventeen cases of testicular teratoma in children admitted to Xiamen Maternal and Child Health Hospital were reviewed.Their clinical features,twodimensional ultrasonography and color Doppler imaging were analyzed and compared with the results of operation and pathology.Result:High frequency color Doppler ultrasonography was performed before operation.The ultrasonographic manifestations of testicular teratoma were different,the size and echoes of teratoma varies.All the pediatric patients underwent surgical removal or resection.The pathological examination showed that there were 4,11 and 2 cases of immature,mature and malignant teratoma respectively.The boundary of benign testicular teratomas was clear,and there was no or occasional punctate blood flow signal in and around them.The boundary of malignant teratomas was blurred,and there were abundant and disordered thick strip blood flow signals in and around them,which were quite different.Conclusion:Although the sonographic features of testicular teratoma in children are varied due to the consistency and concentration of its components,it also has high specificity.Ultrasound can be used as an important method for clinical diagnosis of testicular teratoma.
