三例甲状舌管癌的影像学诊断及文献复习

摘要
目的探讨甲状舌管癌(TDCa)的影像学特征,提高鉴别诊断能力。方法回顾性分析3例手术病理证实的TDCa,结合既往文献,分析TDCa患者的影像学特点。结果 3例TDCa均位于舌骨水平上下,病理类型均为乳头状癌,影像学检查均表现为囊实性病灶,可见微钙化,增强扫描为实性成分和囊壁强化。结论 TDCa是由残留的甲状舌管发生癌变所形成恶性肿瘤,病理类型多为乳头状癌,囊实性病灶,增强扫描后实性成分和囊壁强化,肿物内含钙化灶为其特征性影像学表现,有助于明确诊断。
Objective To investigate the imaging features of thyroglossal carcinoma to increase the accuracy of its diagnosis.Methods A retrospective analysis was conducted for imagings of three cases of thyroglossal carcinoma confirmed by surgical pathology to analyse the imaging features of thyroglossal carcinoma.Results Three cases of lesions all located near the level of the hyoid bone.All three cases were diagnosed as papillary carcinoma.The imaging findings showed cystic solid lesions with microcalcifications and three cases showed positive enhancement of solid ingredients and cystic wall.Conclusion Thyroglossal duct carcinoma is a malignant tumor raised from the malignant degeneration of remnant thyroglossal cyst.The most common histological type is papillary carcinoma.It is a solid cystic lesion.The enhanced scanning shows positive enhancement of solid component and cystic wall.Calcification is a characteristic imaging finding.These imaging features help to confirm the diagnosis of papillary thyroglossal carcinoma.
