血清PCT、IL-6和hs-CRP早期诊断新生儿肺炎的临床价值研究

摘要
目的探讨血清降钙素原(PCT)、白介素-6(IL-6)和超敏C-反应蛋白(hs-CRP)早期诊断新生儿肺炎的临床价值。方法选取2016年5月—2018年5月我院收治的110例新生儿肺炎患儿(观察组)和同期于我院正常分娩的60例足月健康新生儿(对照组)作为研究对象,分析二组新生儿的临床资料,所有新生儿均行血清PCT、IL-6和hs-CRP等相关临床指标的检测与检查,记录所有新生儿性别、出生日龄等一般临床资料并比较不同新生儿上述临床资料及指标水平的差异,分析不同临床指标对新生儿肺炎的早期诊断价值。结果 (1)根据病原学检查结果可知,110例新生儿肺炎患儿中有62例为细菌性感染,占56.36%;(2)新生儿肺炎两组患儿血清PCT、IL-6和hs-CRP水平均显著高于正常新生儿,而细菌性感染新生儿肺炎患儿其血清PCT、IL-6和hs-CRP水平均明显高于非细菌性感染新生儿肺炎患儿(P<0.05);(3)细菌性感染新生儿肺炎患儿治疗后血清PCT、IL-6和hs-CRP水平均较治疗前显著下降(P<0.05),但治疗后的血清PCT、IL-6和hs-CRP水平与对照组新生儿比较均无差异(P>0.05);(4)细菌性感染新生儿肺炎患儿其血清PCT、IL-6和hs-CRP阳性率均显著高于非细菌性感染新生儿肺炎患儿(P<0.05);(5)经Spearman等级相关分析得,血清PCT、IL-6与hs-CRP以及IL-6与hs-CRP均呈正相关(P<0.05);(6)血清PCT早期诊断新生儿肺炎的灵敏度和特异性均较IL-6和hs-CRP诊断高,且当血清PCT>1.0μg/L时较高,但血清PCT、IL-6和hs-CRP联合检测早期诊断新生儿肺炎灵敏度和特异性均更佳,分别为93.8%和90.6%,差异均有统计学意义(P<0.05)。结论血清PCT、IL-6和hs-CRP在新生儿肺炎患儿中均以较高水平表达,其中PCT早期诊断新生儿肺炎较IL-6和hs-CRP好,但PCT、IL-6和hs-CRP联合检测早期诊断新生儿肺炎具有更高的灵敏度和特异性,值得作为临床上早期诊断新生儿肺炎的首选检查方案之一。
        Objective To investigate the clinical value of serum procalcitonin(PCT),interleukin-6(IL-6)and high-sensitivity C-reactive protein(hs-CRP)in the early diagnosis of neonatal pneumonia.Methods From May 2016 to May 2018,one hundred and ten cases with neonatal pneumonia(observation group)and sixty full-term healthy newborns delivered in our hospital during the same period(control group)were selected.The clinical data of all neonates were retrospectively analyzed.All neonates were tested for PCT,IL-6 and hs-CRP.The general clinical data of all neonates,such as sex and age of birth,were recorded.The differences of the above clinical data and index levels among different neonates were compared,and the early diagnostic value of different clinical indicators for neonatal pneumonia was analyzed.Results(1)According to the results of etiological examination,62 of 110 neonatal pneumonia cases were bacterial infection,accounting for 56.36%;(2)The levels of PCT,IL-6 and hs-CRP in serum of the neonatal pneumonia group were significantly higher than those of normal neonates,while the levels of PCT,IL-6 and hs-CRP in serum of the bacterial pneumonia group were significantly higher than those of non-bacterial pneumonia group.(3)The levels of serum PCT,IL-6 and hs-CRP in children with bacterial pneumonia after treatment were significantly lower than those before treatment(P<0.05),but the levels of serum PCT,IL-6 and hs-CRP in children with bacterial pneumonia after treatment were not significantly different from those in control group(P>0.05);(4)The positive rates of serum PCT,IL-6 and hs-CRP in children with bacterial pneumonia were significantly higher than those in nonbacterial pneumonia.(5)Spearman grade correlation analysis showed that serum PCT,IL-6,hs-CRP and IL-6 were positively correlated with hs-CRP(P<0.05);(6)The sensitivity and specificity of serum PCT in early diagnosis of neonatal pneumonia were higher than those of IL-6 and hs-CRP,and were higher when serum PCT>1.0μg/L,but the sensitivity and specificity of combined detection of serum PCT,IL-6 and hs-CRP in early diagnosis of neonatal pneumonia were better,93.8% and 90.6%,respectively,with statistically significant differences(P<0.05).Conclusion Serum PCT,IL-6,and hs-CRP are all highly expressed in neonates with pneumonia.For single detection,the clinical value of PCT is better than that of IL-6 and hs-CRP,while the combined detection of PCT,IL-6 and hs-CRP has a higher sensitivity and specificity in early diagnosis of neonatal pneumonia,which is worthy of recommendation.
