高危儿联合门诊随访对早产儿智能和生长发育的影响

摘要
目的通过研究早产儿在高危儿联合门诊的随访,分析其对早产儿智能和生长发育的影响。方法回顾性分析2015年唐山市妇幼保健院高危儿联合门诊系统随访的110例早产儿档案资料作为观察组,将2013年高危儿联合门诊尚未成立前唐山市妇幼保健院儿科门诊随访的108例早产儿档案资料作为对照组。比较纠正胎龄18月龄时观察组与对照组的贝利婴幼儿智能测验结果、身高、体重和头围;分析早产儿体重低下、生长迟缓、智力发育迟滞、运动发育迟滞在二组中所占的比例。结果观察组纠正胎龄18月龄时早产儿的智力发展指数、心理运动发展指数、身高、体重、头围均高于对照组,差异均有统计学意义(均P<0.05)。观察组中的体重低下、生长迟缓及智力发育迟滞、运动发育迟滞的发生率明显低于对照组,差异有统计学意义(均P<0.05)。结论儿童保健科、神经康复科和儿科合作的高危儿联合门诊无论在早产儿的智能发育还是生长发育的早期干预效果中均优于单一的儿科门诊,在医院中推广此模式有力于早产儿的智能发育和生长发育。
Objective To study the follow-up of premature infants in high-Risk outpatient clinic,and to analyze its impact on the intelligence and growth of premature infants.Methods A retrospective analysis was made on the records of one hundred and ten premature infants who were followed up by the high-risk outpatient service system of Tangshan Maternal and Child Health Hospital in 2015.108 cases of premature infants who were followed up by the pediatric outpatient service of Tangshan Maternal and Child Health Hospital before the establishment of the high-risk outpatient service in 2013 were taken as the control group.The results of intelligence test,height,weight and head circumference of Bailey infants in the observation group and the control group at 18 months of gestational age were compared,and the proportion of low weight,growth retardation,mental retardation and motor retardation of premature infants in the two groups was analyzed.Results The intelligence development index,psychomotor development index,height,weight and head circumference of preterm infants at 18 months of gestational age in the observation group were higher than those in the control group,and the difference was statistically significant(P <0.05).The incidence of low body weight,growth retardation,mental retardation and motor retardation in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The combined outpatient clinic of high-risk infants cooperated by children′s health department,neurorehabilitation department and pediatrics is superior to the single pediatric outpatient clinic in the early intervention effect of intelligence development and growth development of premature infants.Promoting this model in hospitals is effective for intelligence development and growth development of premature infants.
