Prognostic significance of cardiac enzyme elevations detected through systematic screening in clinical trials

The prognostic significance of cardiac enzyme (CKMB) elevations after successful coronary intervention was examined in GUSTO II (comparing heparin and hirudin in acute MI and unstable angina) and IMPACT II [elevating the antiplatelet agent Integrelin in coronary angioplasty (PTCA)]. The IMPACT II analysis included 2426 patients with complete procedural data and available post-intervention CKMB levels. 1795 patients (74 % ) had normal post-procedural enzymes. Of 631 (26 % ) with elevated CKMB in the post-procedure period, 531 met criteria for angioplasty success and 100 were classified as unsuccessful (i.e., residual vessel blockage). Patients with elevated CKMB after successful revascularizations had fewer events than patients with unsuccessful interventions but more than patients with normal post-procedural CKMB. CABG and PTCA include urgent/emergency procedures only. Results from GUSTO II and outcome data to six months will also be presented. The p values compare normal CKMB with success/CKMB↑.
